口腔与外界、鼻咽腔相通，口腔内的软、硬组织形成了从有氧到无氧的特殊微环境，加之其适宜的温度、湿度和营养，为许多微生物的定居、生长和繁殖提供了条件。有报告指出从牙周袋中可分离出400种以上的微生物。
菌斑细菌及其产物是引发牙周疾病的始动因子。在为数众多的菌斑微生物中，究竟是少数细菌还是全部细菌导致了牙周疾病的发生发展，迄今仍是一个悬而未决的问题。非特异性菌斑学说和特异性菌斑学说代表了两种截然相反的认识。非特异性菌斑学说强调菌斑细菌的量，认为牙周疾病的发生、发展是菌斑内总体微生物综合效应的结果，因此如何量化菌斑和牙石是非特异性菌斑学说必然要关注的问题。
特异性菌斑学说强调菌斑中细菌的质，认为仅有少数具毒力和能损害宿主防御功能的特殊致病菌，才对牙周疾病的发生、发展起关键作用。特异性菌斑学说的提出依赖于细菌培养和检测技术的进步。
1.培养法（图1-211）
细菌培养法是微生物学检查的“金标准”。可用于细菌分类、生物学特性、药敏、致病性、细菌毒力等研究。由于牙周炎是厌氧菌为主的感染，其培养条件要求比较高，所需周期比较长。从龈下菌斑中培养出某种细菌，可以确信其确实存在。但反之却不能成立，由于体外很难模拟出体内的实际情况，从龈下菌斑中没有培养出某种细菌，不能得出龈下没有这种细菌的确切结论。

在我国牙周疾病是最常见的一种口腔疾病。第三次全国流行病学调查显示35～44岁年龄组牙周健康率仅为1 4.5％。牙龈出血、牙周袋的检出率、附着丧失≥4mm检出率分别为77.3％、40.9％和n38.9％。65～74岁年龄组牙周健康率仅为14.1％。牙龈出血、牙周袋的检出率、附着丧失≥4mm检出率分别为68.0％、52.2％和71.3％。这些数字表明，我国牙科医生每天接诊的成年患者中或多或少都存在有牙周的问题，牙周检查技术也是每位牙科医生应该了解的一项基本技术。
