王宏伟，北京协和医院皮肤科教授，主任医师。为中华医学会皮肤病分会美容组会员，中国医师协会皮肤病分会激光组会员，国家药监局专家组成员，北京卫生局医疗美容质控中心专家。
主要从事皮肤色素性疾病、血管性疾病的研究。负责北京协和医院皮肤激光门诊工作，积极开展激光治疗新技术如用点阵激光治疗多种皮肤疑难疾病，目前已经申请国家专利5项。独立开展多项新的诊断技术，如白癜风、黄褐斑的紫外荧光诊断，光敏剂紫外荧光诊断等。现在从事皮肤超声等新技术的研究工作。

花斑糠疹可出现白色斑，与白癜风如何区别？
花斑糠疹，又称花斑癣、汗斑。皮肤损害以淡白色为主，皮损大多数呈圆形或卵圆形斑，边缘模糊，表面往往有很多微细的鳞屑。患者皮肤表面有折光性，由于是细菌感染性的皮肤病，所以可直接通过镜检找到真菌。花斑糠疹主要出现在皮脂腺发达部位，如颈、前胸、背部与上肢。白癜风是一种色素障碍性皮肤病，色素脱失为点状或者连成一片的白斑，边沿可有色素沉着，表面无鳞屑，白癜风不会由于出汗过多病情加重，在显微镜下也找不到真菌。那么，白癜风有什么特点呢？①白癜风是一种色素障碍性皮肤疾病，不是血液病，更不是传染病。白癜风是由于皮肤和毛囊内黑素细胞减少或消失等原因，导致黑素天生的进行性减少或消失，从而引起的局限性或泛发性脱色素病变。②白癜风不是传染病，本身没有传染体，因此我们在日常生活中和白癜风患者的正常交往是不会也不可能传染的。③长期心理压力大、精神紧张会易发白癜风，因此我们在日常生活中要保持乐观的生活态度，适当减轻轻压力，避免精神压力过大，都可以在一定程度上减少白癜风的发作。炎症后白斑等继发性白斑与白癜风如何区别？
继发性白斑有时易与白癜风相混淆，鉴别的要点是继发性白斑都有原发病史或是与原发病同时存在，而且白斑一般发生在原发病作用部位。炎症后白斑是指发炎部位继发性色素减少，见于多种炎症性皮肤病。其发生原因可能是损害处黑素细胞消失，如烧伤或溃疡愈后的瘢痕处及红斑狼疮、扁平苔藓和硬化萎缩性苔藓处的色素减退斑；也可能是角朊细胞分裂加快，黑素细胞内成熟黑素体输入角朊细胞内的数量减少，或表皮角朊细胞更换时间缩短，黑素体在角朊细胞中降解障碍，如银屑病、玫瑰糠疹、白色糠疹、脂溢性皮炎及神经性皮炎后的色素减少。有时白癜风与炎症后白斑在一些病例不易区别，可用wood灯帮助检察鉴别。在wood灯光线下白癜风皮损反光为纯白色，而多数炎症后白斑因含有一些黑素，可吸收一部分光线，所以检查时不像白癜风那样纯白。
黏膜硬化萎缩性苔藓与长在黏膜部位的白癜风如何区别？
首先，白癜风与硬化性萎缩性苔藓在好发部位上有一定的差别。白癜风无一定好发部位，皮损形态大小不一，可以融合成较大的斑片。白斑不高出皮肤，周边有色素增加。而硬化萎缩性苔藓好发于肛门、生殖器，亦可分布于颈侧、锁骨上窝、胸背上部、腹部特别是脐周、腋窝、手腕屈侧。皮损为象牙白色丘疹、黄色或珍珠母状。有时丘疹表面有小的角质栓塞性黑点，用力剥除后，留下一幽谷状凹陷，可融合成各种大小与形状的斑块，皮损边缘呈红色，边界清楚，光滑。到晚期成羊皮纸样外观。此时与白癜风鉴别可能会有困难。
其次白癜风为黑素细胞减少或消失，硬化萎缩性苔藓为皮肤角化过度，毛囊口和汗管口有角栓。颗粒层变薄但完全存在。在角化过度明显处，颗粒层可增厚，棘层减少，表皮突明显减少或完全消失。基底层细胞不呈圆柱状而为立方形，液化变性，表皮下和真皮上1／3处胶原纤维水肿和均质化，不易着色。同时有毛细血管扩张，弹力纤维减少或消失。充分发育的皮损，在真皮中层血管周围可见有带状或片状淋巴细胞浸润。
