Author Description

王翰章，男，北京人，1949年毕业于华西协合大学，获美国纽约州立大学博士学位。历任华西大学、四川医学院、口腔颌面外科学教授、科主任、口腔医学系主任、口腔医院院长、四川医学院教务长、副院长、华西医科大学顾问、卫生部医学科学研究委员会口腔专题委员、高等医药院校教材编委会委员、国家自然科学基金委员会学科组评审员、中华医学会四川分会常委、口腔专业委员会主任委员、中华口腔医学会顾问、口腔颌面外科学专业委员会顾问、《中国医学百科全书》编委会委员、《口腔医学卷》主编、《中国口腔医学年鉴》主编、《华西口腔医学杂志》主编及有关7种专业杂志特邀编委。现任四川大学华西口腔医学院口腔颌面外科学教授，博士研究生导师，国际牙医师学院院士，为中国口腔颌面外科学创建人之一。长期致力于口腔颌面外科学医疗、教学、科学研究工作，对于颌面部损伤与畸形整复外科学、颌骨血供动力学、皮肤组织血供、人工骨生物学基础、口腔医学信息学的研究作出了突出工作。已发表论文70余篇、编写专著10部，1978年获全国卫生科学大会奖，先后获省、部级科技进步奖多项，在救治伤员中，荣立战功。获国务院颁发的“对医疗卫生事业作出突出贡献者”证书及国家教委颁发的“从事高校科技工作四十年成绩显著”荣誉证书。书画作品曾在全国大赛获奖，并收录于国内外多部名人录中。1990年获全国优秀教师奖，2005年获中国口腔颌面外科华佗奖，2006年获中国口腔颌面外科建设与发展杰出贡献奖，2007年获杰出口腔医师奖和中国医师奖，享受国务院政府特殊津贴。
周学东，女，四川南充人。1987年毕业于华西医科大学，获医学博士学位。随后赴丹麦奥尔胡斯皇家牙学院研修，1988年回国。现任四川大学教授、博士生导师、华西口腔医学院院长、口腔疾病研究国家重点实验室主任、国务院学科评议组成员、教育部高等学校口腔医学教学指导委员会主任委员、中华口腔医学会副会长、《中国口腔医学年鉴》、《华西口腔医学杂志》主编、《中华口腔医学杂志》副主编。主要研究龋病病因及防治技术，先后主持10余项国家及部省级科研课题，发表学术论文80余篇，主编专著6部，获省部科技成果奖6项。曾获第四届中国青年科技奖、卫生部有突出贡献的中青年专家、四川省有突出贡献的中国博士、四川省杰出创新人才奖和教学名师。

Book Abstract

第一章拔牙学
牙拔除术（extraction of teeth）是口腔颌面外科最基本、最常用的手术，也是治疗某些牙病和由其引起的局部或全身疾病的积极手段。
牙拔除术适应证及禁忌证的掌握是否得当是治疗成功与否的关键之一。首先必须强调的是：牙齿是人体重要的咀嚼器官，对牙齿的拔除应持慎重的态度。其次应明确的是：无论是适应证还是禁忌证的确定是相对的，是随着科技发展和条件变化而不断改变的，如随着口腔各学科临床业务的发展，过去认为无法保留的牙齿现在常常可以通过积极的治疗得以保存，并恢复其功能或为义齿修复创造良好的条件。另一方面，一些过去未被认识的病灶牙，现在由于其与某些全身性疾病之间关系的确认，而成为拔牙的适应证。这就要求口腔医师不仅要熟知本专业基础理论及临床知识，还要及时了解本专业各科最新发展动向及相关各科的医学基础理论和临床知识。
牙拔除术不可避免地要给口腔软、硬组织带来不同程度的创伤，且手术在口腔有菌环境中进行，不可避免地会出现一些局部和全身的反应，如局部肿胀、疼痛，和血压、心率、体温、心理变化以及菌血症等。甚至还可能出现一些严重的并发症，如诱发心血管疾病、急性肾炎、甲状腺危象等。因此，应严肃对待牙拔除术，不断提高技术技巧，减少创伤，同时注意消毒灭菌，恪守手术操作原则，做好术前术后处理，以减轻手术不良反应，预防感染及其他并发症的发生。
牙拔除术对患者还可产生明显的心理影响。多数情况下患者会对拔牙产生恐惧和精神紧张。因此，需要了解患者的心理状况，尽量减轻或解除他们对拔牙术的各种顾虑。
