Book Abstract

四、脏腑分证的提出
早在《中藏经》中就专门阐发了辨脏腑虚实寒热的病证。其中，最有代表性的是《论五脏六腑虚实寒热生死逆顺之法》等11篇。例如辨肝脏的脉证，首先明确肝脏的生理属厥阴，主春气，与少阳胆互为表里，并以“嫩而软，虚而宽”描写肝主柔和、疏泄的特征。其次分析肝主弦软而有弦长、弦软、弦实、弦虚之不同，及其所主太过、不及的病变等。自《中藏经》第一次以脉证为中心分述五脏六腑寒热虚实病证之后，唐代孙思邈的《备急千金要方》、宋代钱乙《小儿药证直诀》咸宗之，而为脏腑辨证之所本。孙思邈继《中藏经》、《脉经》之后，在《备急千金要方》中将多种内伤杂病用10卷的篇幅分属五脏六腑进行论治。如记载云：“皆备述五脏六腑等血脉根源，循环流注，与九窍应会处所，并论五脏六腑等轻重大小长短阔狭，受盛多少，仍列对治方法，丸散酒煎汤膏摩熨及灸针孔空，并穷于此矣。”意即是说，对每个脏腑病证，从生理、病理、治法、方药等方面一一阐述，形成体系。具体在诊断上，于五脏六腑各卷中列有脉论一节；在辨证上，各脏腑均有寒热虚实之分，并有五脏之劳，筋、脉、肉、气、精、骨等六极，以及髓、脉、肉、皮、骨等五体虚实病证；在疾病归类上，各病证分属各脏腑，每病证之后详列方治。这种以五脏六腑为纲，寒热虚实为目，包括五劳六极、坚瘕积聚等，分成若干单元进行论述的疾病分类法，颇为易学易用。
宋代钱乙虽然把五脏辨证的方法运用于儿科临床，但其诊断、辨证的思路，仍属于内科范畴。他结合儿科特点，先列“五脏所主”，即五脏的主证，辨别其虚实，并以虚实寒热为论治的准则。其中把“风、惊、困、喘、虚”归纳为肝、心、脾、肺、’肾五脏的主要证候特点，用虚实寒热来判断脏腑的病理变化，用五行来阐述五脏之间以及五脏与气候时令之间的相互关系，立五脏补泻诸方作为治疗的基本方剂。
钱氏不仅强调五脏证治，而且从整体观出发，认为五脏之间可以相兼为病，并应用五行生克乘侮理论，来辨别五脏相兼病证的虚实，判断其预后，以及采取相应的治法。钱氏的这些观点被后世用于内科，其创立的不少方剂亦为内科引申应用，促进了内科学的发展。
