Book Abstract

设想一下，如果我们关于心脏病的医学知识是这样的：我们能够较准确地诊断出心脏病，但是对于锻炼、胆固醇、盐、脂肪、压力、疲劳这些因素对心脏病的影响一无所知；那么，那些被诊断出心脏病的人就会尝试所有有助于他们康复的方法。一些人可能停止一切锻炼，而一些人可能会剧烈地运动。一些人可能会回避有压力的情境。一些人可能会服用降压药，但他们并不知道自己不健康的饮食习惯会使药物失效。很多人会过早死亡，偶尔有些人会好转。没有严谨的、可控的科学研究，医生们无从知晓是什么原因导致了一些人的死亡，又是什么原因导致了一些人的康复。

陷入抑郁的人仿佛已经深陷灭顶之灾而无力自拔。他们在生活中很努力，试图去解决自己的问题；但他们的努力是徒劳的，因为他们缺乏必要的技巧来帮助处于深渊中的自己。抑郁症实际上是自我的各个部分间的斗争。抑郁症患者被自己的阴影、灵魂、自我的各部分拖入了深渊之中，这些东西无法整合也无法释放。他们越是努力，越是清楚如何去做，情况反而变得越糟。当他们的爱人试图以平常的方式、一种关怀爱护的自然表达帮助他们时，他们会拒绝。而抑郁症患者随即会感到更加内疚并失去控制。

很显然，对于我来说，这是一本个人色彩强烈的书。我想要挽救潜在自杀者的生命，我想要缓解抑郁症患者的痛苦。相比于多年前，我的母亲和我自己所能得到的帮助，现在可以做的事情很多很多。心理治疗和药物治疗给予了每个人希望。对于那些最了解抑郁的人来说，学会自我控制的技巧，学会沟通及自我表达的技巧，学会挑战对自己和世界的固有看法，将使他们有机会获得愉快的人生。
当我在社区心理健康诊所工作的时候，让我感触颇深的是许多人不知道他们自己患有抑郁症。通常，人们之所以急切地打电话寻求帮助，不是因为他们感到自己病了，而是他们的生活出了一些问题：孩子不听话，婚姻出现危机，工作上遇到了困难。但通常，治疗师很快就能发现求助者已经患有一段时间的抑郁了；家庭冲突和工作问题只是抑郁症表现出的结果而不是原因。如果我们能够尽快帮助他们，他们的生活就不会像现在这样糟糕。抑郁症患者觉得生活中几乎没有快乐，没有希望，没有抱负；感到好像被卡住了，无能为力；经常感到悲伤；而他们觉得这种感觉是正常的。但实际上并不是这样的。
