《2015心脏病学进展》：
一、定义
儿童高血压指根据统计学标准，结合年龄、性别和身高因素，收缩压超过整体测量范围95%界值。为和成人高血压描述一致，血压处于90%-95%界值时称之为临界高血压。
二、流行病学
儿童和青少年高血压的诊出率在不断上升。大量人群研究显示高血压儿童在成人后罹患高血压的可能性大大增加。年龄较小的高血压儿童多为继发性高血压，但随发病年龄的增加，原发性高血压更为多见。17～18岁时血压上升较明显，多达到成人高血压诊断标准。血压的升高在出生后数周及青春期较为明显。血压值和性别、身高和体质指数相关。肥胖是儿童高血压的强危险因素。目前尚无不同种族间儿童高血压发病差异的充足研究结果，但根据现有的一些研究结果，黄种人及黑种人儿童的血压较白种人儿童的更高一些。儿童高血压的遗传性约有50%。86%的青少年高血压患者有高血压家族史。母乳喂养和儿童期高血压的发生呈负相关。
三、儿童血压的测量方法和参考值
有必要规范测量步骤以正确测量血压，明确儿童和青少年血压情况。正确的测量方法：所测对象处于安静、放松状态，坐在有靠背的椅子上，右上肢位于心脏水平。袖带尺寸需适合儿童上肢。袖带偏小会高估血压水平，偏大则会低估血压水平。充气部分应位，于肩峰和肘突连线中部，宽度约为上肢周长的40%。如对袖带尺寸有疑问，建议使用稍大的袖带。袖带充气气囊的长度应达到臂部周长的80%～100%。听诊器应放置在袖带下缘肱动脉近肘窝处。测量时汞柱上升直至脉搏音消失处以上20mmHg处，然后以2～3mmHg/min的速度下降。收缩压即出现第一次脉搏音的压力值，舒张压即脉搏音消失时的压力值。需要在不同场合测量3次以上方能诊断高血压。目前水银血压计因水银存在毒性慢慢减少应用，但波形测量装置是儿童血压存在测量误差的原因之一。基于波形测量装置所诊断的高血压必须采用无汞压力计结合听诊来进一步确诊。无汞压力计需要每6个月校正1次。3岁以上儿童须每年随访一次血压。所有儿童在以下增加高血压风险的情况下需测量血压：重症监护，肾脏疾病，使用已知能升高血压的药物，存在颅内高压。
几乎所有研究都采用了美国血压标准。特别需要注意的是，美国标准中明确指出了测量时的听诊方法。
成人高血压的诊断中采用了24h动态血压监测。该检查有助于排除“白大衣性高血压”（诊室内血压升高而平时血压正常）及“隐匿高血压”（诊室内血压正常而平时血压升高）。该技术还可以明确是否存在昼夜血压变异。儿童中该检查因缺乏大规模人群的参考值而诊断价值不大。目前AHA和欧洲高血压协会推荐的正常值是根据相对较小样本的研究而得出的。该研究得出了不同性别、年龄和身高儿童昼夜血压的75%、90%和95%界值。
家庭血压自测在血压监测中较新且较重要。目前这方面的数据极少。参考值来源于一个入选800例儿童的研究。正确测量血压需要连续3d在早晨及晚上进行两次血压测量，且两次测量之间间隔数分钟。
