Author Description

张顺祥，1982年12月获公共卫生学士学位后，曾任山西医科大学公共卫生学院流行病学教研室助教、讲师和副教授，期间获得西安医科大学流行病学硕士学位，1995年10月调入深圳市卫生防疫站（现更名为深圳市疾病预防控制中心），任副主任医师和主任医师至今发表论文80余篇，涉及现场流行病学、传染病流行病学，决策分析和卫生经济学以及计算机技术的流行病学应用等研究领域，参编学术专著12部，负责或参与完成多项科研项目并获得科技成果9项

迈克尔（Michael B.Gregg）博士，曾任美国疾病预防控制中心病毒性疾病科主任10年，在美国疾病预防控制中心主办的发病死亡周报（WWMR）任主编21年，担任美国疾病预防控制中心流行病学项目办公室副主任19年发表论文60余篇，涉及流行病学现场调查和公共卫生等领域，主编的《现场流行病学》第1版和第2版分别于1996年和2002年出版，第3版面世后不久，于2008年7月病逝。

随着商业交易记录计算机化的日益普及，我们应该清醒地意识到，在暴发调查中使用这些数据的潜在价值。特别是大型购物俱乐部和超级百货商场，可能存储有顾客或会员极其详细的购物信息。能够利用这些信息是件好事，因为这些超级大店服务的人群相当庞大，如果没有遵守严格的卫生标准，就容易造成食源性疾病的病原体广泛传播。计算机储存的大量消费信息将对处理广泛传播的食源性疾病暴发事件大有帮助。门罗县的现场流行病学调查，发现了一个有趣的现象，在给焙烤食品涂抹糖时，经过手污染点心，造成甲肝传播这样一个传播机制。1968年，在密歇根州WestBranch发生的一起食源性疾病暴发调查中，也曾经发现过类似的传播机制。由此看来，尽管技术与计算机正不断地改变着世界，但是某些疾病传播的方式却持久不变。五、行政管理数据用于公共卫生监测时的缺陷：1991年纽约州一起虚假的霍乱暴发疫情20世纪90年代，利用计算机管理卫生保健系统的数据取得了引人注目的发展。管理性数据，例如医疗卫生保险账单数据，是可以随时获得的电子化信息来源之一，分析这些数据能帮助控制卫生服务的费用。现场流行病学工作者早已学会利用计算机系统中的公共卫生数据，并把该数据的利用作为实施公共卫生监测的可能方法。这类系统就算不是全人群数据，至少覆盖了大量的人口。这类数据一般也容易得到，即使费点周折也能获取（通常需要一些官方文件允许才能获得），而且，数据可以直接用计算机分析。但是，为了公共卫生目的而利用该类数据时，必须格外小心。下面这个例子说明了这一点。
1993年，纽约州卫生局的传染病处检查了居民健康电子数据库，希望把这部分信息利用起来，作为现存的法定报告疾病监测系统的补充。数据来自于纽约州财政机构，涉及95万名纽约州和纽约市的职员、退休人士及其家属1991年全部的健康保险理赔资料。按照分析计划，要从保险理赔数据库里核查法定传染病的ICI）_9编码，并且把分析结果与州传染病监测系统的病例情况作对比。表20-1显示的是1991年保险数据库中4种法定报告传染病的理赔申请数量，这4种疾病是：霍乱、伤寒、其他沙门菌感染和志贺细菌性痢疾。每10万人这几种法定传染病的保险理赔率与全州传染病监测系统测算出来的年度患病率进行对比，结果保险数据得出的沙门菌感染和志贺细菌性痢疾的理赔率低于根据监测数据推导的患病率，但是在率的大小次序上是一致的。而伤寒的两种率是一致的。再看看霍乱的保险理赔数据，有35例理赔单的诊断为霍乱，即ICD编码为01，而通过传统的监测系统拿到的数据中却没有发现霍乱病例报告。
为进一步核实保险理赔单编码的真实性，调出保险记录查看，检查这些病例是否做了大便培养，因为大便培养是确诊这几种疾病所必需的。可以通过保险数据库中有没有填写大便培养费用一项判断。在沙门菌感染的理赔数据中发现，大便培养检测率仅为29％，而霍乱、伤寒和志贺细菌性痢疾的检测率分别为3％、0和13％。此结果提示，健康保险理赔数据库中疾病诊断的正确性值得考虑。
