Book Abstract

【作用与用途】青霉素通过抑制细菌细胞壁合成而发挥杀菌作用，对溶血性链球菌等链球菌属，肺炎链球菌和不产青霉素酶的葡萄球菌具有良好的抗菌作用。对肠球菌有中度抗菌作用。淋病奈瑟菌、脑膜炎奈瑟菌、白喉棒状杆菌、炭疽芽孢杆菌、牛型放线菌、念珠状链杆菌、李斯特菌、钩端螺旋体和梅毒螺旋体对本品敏感。本品对流感嗜血杆菌和百日咳鲍特菌亦具一定抗菌活性，其他革兰阴性需氧或兼性厌氧菌对本品敏感性差。本品对梭状芽孢杆菌属、消化链球菌厌氧菌以及产黑色素拟杆菌等具良好抗菌作用，对脆弱拟杆菌的抗菌作用差。适用于溶血性链球菌、肺炎球菌、脑膜炎球菌、敏感金葡菌等感染，亦应用于草绿色链球菌和肠球菌所致的心内膜炎，气性坏疽，厌氧球菌感染，炭疽，梅毒，钩端螺旋体病，淋病等。可与特异性抗毒素联合应用于破伤风及白喉，并可用于风湿病的预防。
【体内过程】肌内注射后，0.5小时达到血药峰浓度（Cmax），肌内注射1×106U（600mg）的峰浓度为2×104U/L（12mg/L）。新生儿按体重肌内注射青霉素2.5×104U/kg（15mg/kg），经0.5~1小时后，平均血药浓度约为22mg/L，12小时后即降至9.6~19.2mg/L。成人每2小时静脉注射本品2×106U或每3小时注射3×106U，平均血药浓度约为19.2mg/L。于5分钟内静脉注射5×106U（3g）青霉素，给药后5分钟和10分钟的平均血药浓度为400mg/L和273mg/L，1小时即降至45mg/L，4小时仅有3.0mg/L。本品广泛分布于组织、体液中，胸、腹腔和关节腔液中浓度约为血清浓度的50%。本品不易透人眼、骨组织、无血供区域和脓腔中，易透入有炎症的组织。青霉素可通过胎盘，除在妊娠头3个月羊水中青霉素浓度较低外，一般在胎儿和羊水中皆可获得有效治疗浓度。本品难以透过血-脑脊液屏障，在无炎症脑脊液中的浓度仅为血药浓度的1%~3%。在有炎症的脑脊液中浓度达同期血药浓度的5%~30%。乳汁中可含有少量青霉素，其浓度为血药浓度的5%~20%。本品血浆蛋白结合率为45%~65%。
