Book Abstract

（一）青霉素
【体内过程】 青霉素口服易被胃酸及消化酶破坏，故不宜口服。通常作肌内注射，吸收迅速且完全。注射后达峰时间为0.5～1.0小时。该药因脂溶性低，主要分布于细胞外液。几乎全部以原型迅速经尿排泄，约10％经。肾小球滤过排出，90％经肾小管分泌排出，为0.5～1.0小时。
青霉素有3种剂型：水溶液的青霉素钠或钾盐，混悬剂普鲁卡因青霉素和油剂苄星青霉素，分别适用于急性或重症感染，轻症患者或预防感染。
【抗菌作用】 青霉素抗菌作用很强，在细菌繁殖期低浓度抑菌，较高浓度杀菌。
对下列细菌有高度抗菌活性：①大多数革兰阳性球菌；②革兰阳性杆菌；③革兰阴性球菌；④少数革兰阴性杆菌；⑤螺旋体、放线杆菌。
【临床应用】 本药肌内注射或静脉滴注为治疗敏感所致感染的首选药。如溶血性链球菌引起的蜂窝织炎、丹毒、猩红热、咽炎、扁桃体炎、心内膜炎等；肺炎球菌引起的大叶性肺炎、脓胸、支气管肺炎等；草绿色链球菌引起的心内膜炎、淋球奈瑟菌所致的生殖道淋病；敏感金黄色葡萄球菌引起的疖、痈、败血症等；脑膜炎奈瑟菌引起的流行性脑脊髓膜炎；也可用于放线杆菌病、钩端螺旋体病、梅毒、回归热的治疗；还可用于白喉、破伤风、气性坏疽和流产后产气荚膜梭菌所致的败血症的治疗。
注意 但因青霉素对细菌产生的外毒素无效，故必须加用抗毒素血清。
【不良反应】
（1）变态反应
注意 为青霉素类最常见的不良反应，在各种药物中居首位。
各种类型的变态反应都可出现，以皮肤过敏（荨麻疹、药疹等）和血清病样反应较多见，但多不严重，停药后可消失。最严重的是过敏性休克。
发生变态反应的原因是青霉素溶液中的降解产物青霉噻唑蛋白、青霉烯酸、6一APA高分子聚合物所致。用药者多在接触药物后立即发生，少数人可在数日后发生。过敏性休克患者的临床表现主要为循环衰竭、呼吸衰竭和中枢抑制。
