肾穿刺检查全称“经皮肾穿刺活体组织检查”，又称肾活检。它是活体获取肾组织标本做病理检查的重要手段。肾活检是诊断肾炎的重要检查方法之一，对确定诊断、探讨临床与病理的联系、决定治疗方案和估计预后都有重要价值。经肾活检后疾病临床诊断的修正率达34％～63％，治疗方案修正率达19％～36％，预后估计修正率达32％～36％。对于内科各种原发、继发及遗传性肾实质疾病而无肾穿刺禁忌证者，均可进行肾穿刺。其具体适应证及禁忌证由专业医生掌握。

肾组织病理检查包括光学显微镜检查、电子显微镜检查及免疫荧光（或免疫组织化学）检查。这多种检查资料的综合，显著地提高了疾病诊断的准确性。所以，肾穿刺活检对确定肾小球疾病诊断、制定治疗方案及判断预后都很有意义，它已成为肾内科常做的一项重要检查。
具体来说，肾穿刺活检对肾小球疾病诊断有如下帮助：
（1）确定是否患了肾炎如体位性蛋白尿与隐匿性肾炎有时在临床上极难鉴别，只有做肾穿刺病理检查才能将二者准确I爰分。
（2）确定是患了哪种肾炎肾小球肾炎可分为原发性及继发性两大类，它们各自又包含着许多不同性质的肾炎，如原发性肾小球肾炎至少就包括有9种病理类型，不同病理类型在疾病进展速度、治疗方案、疗效反应及预后上差别极大，因此做肾穿刺活检将它们准确区分很重要。
（3）确定病变轻重及是否可逆有时疾病的临床与病理表现并不平行，只有做肾穿刺病理检查才能准确了解病变轻重，并了解病变是活动病变（如细胞增生或浸润，具有可逆性）或已是不可逆性病变（如纤维化及硬化）。只有明确上述各诊断问题后，才能准确制定治疗方案及判断疾病预后。
