Author Description

王陇德，中国工程院院士，卫生部原副部长，中华预防医学会会长。现担任第十一届全国人大常委、卫生部“健康中国2020战略研究组”首席专家、卫生部疾病预防控制专家委员会主任委员、浙江大学公共卫生学院院长等职。长期在公共卫生领域从事行政管理、流行病学和公众健康促进专业研究工作，在《中华医学杂志》等国内外学术期刊发表论文100余篇，主编多部专著。曾获国家科技进步二等奖、联合国艾滋病规划署颁发的“应对艾滋病杰出领导和持续贡献”奖及世界卫生组织颁发的结核病控制“高川”奖。为提高国民健康水平，他陆续发表了“中国人需要一场膳食革命”、“中国人需要一场行为革命”、“脑卒中筛查与干预：一项重大的国民保健工程”等科普文章，并出版了《首席专家王陇德谈掌握健康钥匙》科普图书。

5.前列腺与性激素的密切关系
在男性的幼儿时期，前列腺较小。随着男性生长发育，雄激素水平的提高，前列腺可以迅速增大，发挥其应有的生理功能。30岁后前列腺体积稳定，直至45岁以后，腺体若不呈增生状态，则到了老年期，前列腺组织出现萎缩，其体积亦随之缩小。但更多男性随着年龄增加，前列腺体积不断增大，这是因为虽然雄激素随年龄而变化，出现老年时尿内睾酮减少，但血浆内含量自成年到80岁可不变；亦有报道50岁以后血浆睾酮略有下降。
青春期前不会发生前列腺增生，切除睾丸者到老年亦不会发生前列腺增生，有学者对清末太监进行研究，发现他们从小切除睾丸，体内没有足够的雄激素产生，前列腺不发育，也就没有前列腺增大的可能。
由此可以证明，前列腺与性激素关系密切。近年来证明，无论是良性或恶性的前列腺病变，都与激素的作用有关，特别是雄激素。因此，临床应用这一理论，用雌激素治疗前列腺增生和前列腺癌，有一定的缓解作用。
6.前列腺与性的密切关系
前列腺不论在生理上还是病理上都与性有着密切的关系。
（1）首先前列腺是男性内在的性征。当男性第二性征开始发育时，脑下垂体会刺激睾丸成长，此时，睾丸内有大量的精子产生并成长，亦开始大量分泌雄激素，就在这种激素的刺激下，前列腺也开始增大，所以惟有睾丸功能正常运转，前列腺才能正常地发育。一般来说，如果睾丸的功能正常，会持续分泌男性激素--睾酮，前列腺就会不停的增大。30～45岁之间前列腺的大小比较稳定，到了50岁左右，有些人前列腺逐步增大，出现前列腺增生。
（2）男性受到性刺激时，外生殖器官的变化较明显，如阴茎的勃起、胀大粗壮，睾丸提升。而前列腺的变化却深藏体内不为人知。在性高潮时，随着正常的射精，前列腺会有节律地收缩，把前列腺液挤压出去和精囊液等组成精液，排出体外，并获得性快感。因此前列腺参与了性高潮的形成。
（3）当前列腺有病变时，性功能就会受到不同程度的影响。前列腺较常见的疾病有前列腺炎、前列腺增生及前列腺癌，这些疾病都会影响到性生活的质量。
（4）当前列腺因各种原因切除后，一方面前列腺液的减少会导致精液总量的减少；另一方面还可能因前列腺的约束作用失调而出现逆行性的射精，即射精时精液会反方向射入膀胱内，不从尿道口射出，临床上称为不射精症。虽然这种情形对健康无妨，却会使受孕困难，也会使夫妻双方在性活动中有失落感，影响性快感的体验。
