Book Abstract

精神分析治疗
关于心理治疗的技术问题以及疗效的产生机制等问题，精神分析理论中有各式各样的推测，与1946年的情况大不相同。桑德勒在1992年曾总结道：今天，“没有任何一种关于技术的理论是所有精神分析治疗师都能接受的”（Sandler，1992）。然而，直到20世纪50年代，精神分析治疗师还保持着一种共识，即通过对特定的“移情性神经症”的诊断和解释，达到揭示、研究和治疗来访者被压抑的婴幼儿时期形成的心理冲突的目的：这也是当时精神分析的明显特征（ Wallerstein，1989）。精神分析的基本技术手段包括自由联想、患者躺在睡椅上放松的谈话和对梦的分析，治疗要求精神分析治疗师要保持相对的中立态度，不可主动。
20世纪40年代后期，也就是罗杰斯为蒂尔登治疗的时候，精神分析治疗师都明确地认为，可以把“精神分析”与“精神分析治疗”清楚地区别开来，而罗杰斯本人当时还没有对“心理咨询”和“心理治疗”进行区分（ Rogers，1951）。从现在的观点看，“精神分析”与“精神分析治疗”是部分重叠的，而不是完全不同的两种治疗形式。此外，过去一些勉强使用的或被视为不可怀疑的概念也已受到质疑并得到重新解释（Cooper．1987），如“移情性神经症”、阴茎妒忌（Grossman&Stewart，1976）、女性受虐癖（Person，1990）等。另一方面，广义的“移情”（Gill，1982）和“阻抗”等概念（如Schafer，1976）仍然占据着精神分析理论的核心地位。“无意识驱力”也是一个核心概念，这种驱力整合在各种精神内容、症状和行为之中，发挥着隐蔽的作用。

20世纪40年代末期，受过正统训练的精神分析师认为，只有当来访者在强化的情感基础上了解到那些无意识的过程，包括了解到被其遗忘的过去，才可能在精神分析治疗中产生变化。这种强调来访者的顿悟作用的观点可以追溯到弗洛伊德时代。弗洛伊德力求把精神分析法与其他强调建议和劝说的治疗方法区分开来。然而，今天的主流观点是，以单一机制寻求来访者有所改变是注定要失败的。一些精神分析治疗师认为，让来访者重新认识过去对于治疗结果起不了多大作用（ Gill，1982）。

精神分析治疗师之间的观点不同，对于心理功能的认识不同，可分为四个不同的学派，派因把这四个学派称为“内驱力心理学”（psychology of drive）、“自我心理学”（psychology of ego）、“客观关系心理学”（psychology of object relations）以及“自我心理学”（psychology of self）（Pine，1988）。在如何倾听、观察以及组织治疗材料等方面，不同的精神分析治疗学派的观点多少都有不同，并各有其特点。在连续的治疗中，对于不同来访者的精神分析治疗工作都会在不同程度上受到这些不同学派观点的影响。下面，我们从这四个不同学派的观点出发，对蒂尔登个案进行评述。
