（4）对神经肌肉系统的损害：尽管有时缺铁的症状不很明显，但是有证据表明，即使是轻度的缺铁性贫血，也可以影响肌肉的性能。运动最大负荷量、心率、血浆乳酸水平都和贫血的程度成反比。在铁缺乏时，机体的抵御寒冷的能力会下降。偶尔有些病人有神经痛、麻木感。
（5）对上皮组织的影响：长时间的铁缺乏可以造成上皮组织结构或功能的特征性缺陷，特别是指甲、舌咽、口腔、胃肠，这些损害往往在一个病人身上同时出现，但也可以孤立出现。在铁缺乏的病例中，指甲会变脆、易碎或出现纵脊，这些表现很不特异。更具特征性的表现是指甲变扁、变平，最终产生凹面，形成匙状甲，现在临床上已很难看到这种匙状甲。另外，长期重复接触热的肥皂水或其他腐蚀剂也可以形成匙状甲。口腔改变以舌乳头萎缩最常见，表现为舌头灼痛，可以自发或者在进食时发生。占舌2／3的丝状乳头最先萎缩并完全消失。严重者菌状乳头也可受累，使舌面完全光滑呈白色蜡状。这些通常在给予铁剂治疗1-2周后得到逆转。以口角溃疡或龟裂为特征的口角炎，在缺铁时不太特异，也可以发生在维生素B：和维生素B6缺乏时。
（6）免疫和感染：铁缺乏和感染的关系很复杂。铁缺乏至少可以导致免疫应答的两个异常：淋巴细胞介导的免疫缺陷和巨噬细胞吞噬细菌的能力下降。细胞免疫缺陷的证据包括循环T细胞数量下降多达35％，辅助性T细胞和抑制性T细胞都受到影响。
（7）异食癖：是铁缺乏的重要症状。
（8）生殖、泌尿系统：月经过多是铁缺乏的常见原因。
