Book Abstract

桩核冠是利用桩插入根管内获得固位，应用核替代牙冠部分缺损，并最终依靠全冠恢复患牙形态和功能的一种组合式修复体。一般来讲，桩核冠是牙体缺损至一定程度后无法应用嵌体、部分冠、全冠修复而最后选择的修复体。目前，桩核冠是临床上保存和利用残冠残根的主要修复方法。
早在1726年，Fauchard就利用木制桩冠（wood pivot tooth）插入根管内修复患牙，木制桩在吸水膨胀后与根管壁密合达到较好的固位作用，这就是最早的桩冠。而关于“桩”的概念则是19世纪牙科医师在应用管状牙（tube teeth）进行冠桥修复的过程中确定下来的。早期的桩冠通常由木制品或金制作，通过铆接与管状牙形成一个整体，管状牙通过位于髓腔和根管内的桩来取得固位，从而达到固定修复的目的。
1880年，Richmond设计了一种桩、冠一体的金属修复体，以后改进成冠、桩分离，奠定了现代桩核冠的基础。20世纪初，失蜡铸造法开始应用于冠桥的制作，使得金属桩核一度占据着残根残冠修复的主导地位，直到目前为止仍在临床上普遍使用。20世纪中期，人们确定了桩-核-冠系统的组成和作用，这一修复模式最终成为目前残根残冠的主要修复技术。随着社会的进步和文明的提高，人们的要求也在不断提高，传统金属桩核的修复模式已明显不能满足人们追求美观的需求。20世纪后期，随着美学修复的发展，金属材料在口腔中所暴露出来的缺点和无法解决的问题使得学者们对传统桩核冠又重新认识。因此，寻找一种既能保持金属桩核的优点又能弥补金属桩核缺陷的美学桩核材料已成为学者们关注的热点。随后，陶瓷和纤维增强复合树脂桩核材料在树脂粘接技术的支持下相继问世并应用于临床，从而改变了大量使用金属桩核修复的现象。
目前，纤维桩在桩核冠修复领域中占据了越来越大的比例，并有取代金属桩核的趋势。随着现代修复学和口腔材料学的不断发展，桩核冠修复技术也在不断提高和日趋成熟。
