Book Abstract

噻唑烷二酮类药物可显著改善胰岛素抵抗。它与PPAR-y结合，激活该受体，增加肌细胞中胰岛素介导的葡萄糖摄取过程，从而改善葡萄糖的代谢；它与脂肪细胞上的PPAR-γ具有较高的亲和力，进而增加脂联素水平、减小脂肪细胞体积，因而具有改善胰岛素抵抗的作用性。因此，噻唑烷二酮类药物具有改善肌细胞和脂肪细胞等外周组织胰岛素抵抗的作用。
噻唑烷二酮类药物具有保护胰岛β细胞功能。越来越多的基础和临床研究显示，在2型糖尿病患者中，保护胰岛β细胞功能可以减慢和阻止胰岛β细胞衰竭的速度。动物实验资料显示：噻唑烷二酮类药物可降低肥胖ZDF大鼠的胰岛β细胞凋亡，维持β细胞的新生。人胰岛的研究资料表明，吡格列酮通过阻断核转录因子kB（NF-kB）的激活，预防β细胞的凋亡和功能损伤。噻唑烷二酮类药物通过改善胰岛素敏感性的间接作用及激活胰岛内PPAR-y的直接作用来保护胰岛β细胞。
噻唑烷二酮类药物可改善血管内皮细胞功能，改善内皮依赖性的血管舒张和收缩功能。一氧化氮（NO）和内皮素（ET）是血管内皮细胞分泌的相互拮抗的调节血管活性物质，NO松弛血管平滑肌，抑制内皮细胞增殖，而ET则收缩血管，促进内皮细胞增殖，两者的平衡失调可引起血管舒缩功能紊乱。体外实验显示，噻唑烷二酮类药物能增加内皮细胞分泌NO改善心肌血流量，提示它可改善内皮依赖性的血管舒张功能。
噻唑烷二酮类药物具有抗动脉粥样硬化作用。血管内皮细胞功能的损害是动脉粥样硬化形成的早期变化。噻唑烷二酮类药物能改善内皮细胞功能，抑制黏附分子的表达和白细胞的黏附；增加单核及巨噬细胞PPAR-y的表达，减少肿瘤坏死因子-α（TNF-a）、白介素等的表达；可抑制泡沫细胞的形成，抑制动脉粥样硬化斑块的破裂；抑制内皮细胞迁移和凋亡；抑制血管平滑肌细胞的增殖和迁移；延缓动脉粥样硬化的形成。
噻唑烷二酮类药物能抑制炎症反应，改善脂代谢。在肥胖、动脉粥样硬化等病理状态时，炎症标志物如C-反应蛋白（CRP）、TNF-α等炎性介质分泌增加。临床研究显示，经噻唑烷二酮类药物治疗的患者血清炎症标志物（如CRP）显著降低。其机制为噻唑烷二酮类药物激活PPAR-β，后者通过NF-KB而抑制炎症因子的表达，从而达到抑制炎症反应的作用。
