Book Abstract

（8）拔去外套管和导丝置入导管10～15 cm之后退出套管，指压套管端静脉以固定导管，缓慢送导管至预计长度（上腔静脉），拔出导丝，安装连接器、肝素帽。
（9）封管连接注射器，抽回血，注入0.9％氯化钠，确定导管通畅，肝素盐水正压封管。
（10）固定再次对导管人口及周围皮肤进行消毒，固定导管，覆盖无菌敷料。
（11）术后处理与健康指导洗手，整理用物，按要求分类处理垃圾。观察患者有无不适反应。向患者及家属交代置管后的注意事项。
（四）注意事项
（1）穿刺部位必须严格消毒，不得在有感染的部位进行穿刺。
（2）严格无菌操作，疑有导管源性感染，应做导管尖端、皮下段及经导管抽出血培养。
（3）避免反复多次穿刺，以免形成血肿。
（4）如抽出鲜红血液，即表明穿人动脉，应立即拔出，紧压穿刺处数分钟至无出血为止。
（5）应从严掌握适应症，避免发生气胸、血肿、血胸、气栓、感染等并发症。
（6）插管完毕之后应拍床旁x线片以确定导管位置，最佳位置为导管尖端在上、下腔静脉接近右心房开口处。
（7）防止血液在导管内凝集，经常用肝素液冲管。
（五）预防各种并发症
（1）导管错位、导管阻塞、导管折断、意外脱出。
（2）静脉炎、导管相关性感染、过敏反应等。
二、动脉穿刺置管术
（一）概念
该手术经体表穿刺至动脉，插入各种导管到大血管腔内或心腔，用于测定各种生理学参数，同时也可为各种治疗提供直接便利路径。
（二）适应症和禁忌症
1.适应症
（1）采动脉血进行血气分析的患者。
（2）危重及大手术患者进行有创血压监测。
（3）重度休克患者加压输血、输液，提高冠状动脉灌注量及增加有效血容量。
