（二）上下泪小管同时断裂如何吻合
最好应用硅胶泪道引流管对上、下泪小管断端均牢固吻合。上下均断裂时寻找断端相对较为困难，应首先观察断裂的部位，在可能的位置仔细寻找，一般情况下可分别找出上、下泪小管鼻侧断端，插入硅胶导管于鼻腔，然后将断端进行吻合。
术中一定注意上下泪小管的位置关系，不要将上、下位置颠倒。术中上、下泪小管均应进行吻合，对于上泪小管断端不能找到者可单行下泪小管吻合，因为排泪的主要功能在下泪小管。
（三）小儿全麻时的插管注意事项
成人插管后可以通过冲水的方法来观察鼻腔及口腔内有无液体流人，以确定引流管是在正常泪液引流管道内还是在假道内。但小儿基础麻醉后，口腔内是禁止有液体流入的，否则可能因呛咳、窒息而出现生命危险。
1.应选择儿童用泪道引流管，此管直径比成人用管相对细一些，以免造成泪小点及泪小管的过度扩张和撕裂。
2.应尽量选择硅胶泪道引流管，其质地较软，患儿能够更好地耐受。在自下鼻道钩出引流管时，操作要轻柔，尽量不要损伤鼻黏膜，以免发生鼻出血。
3.如选用腰麻管，一定要用带盲端的、直径较细的腰麻管，因其盲端较光滑，出现假道几率相对较少。而剪除盲端的腰麻管因要用斜尖形末端插入，容易在泪小管或总管转弯的地方刺入组织而出现假道。插入时如出现回血说明极有可能已出现假道，需重新更换方向插入。
