第二章超短脉冲激光与飞秒激光器
飞秒激光是脉冲宽度在飞秒（10秒）量级的激光脉冲。在这一章中，我们主要介绍与飞秒激光应用密切相关的光学基础知识和产生飞秒激光的核心仪器一一飞秒激光器的基本原理。我们首先回顾激光和超短脉冲激光有哪些重要特点以及如何以简单的数学表达式来描述飞秒激光脉冲，而后介绍飞秒激光器的一些最基本的参数，并着重阐述产生飞秒脉冲激光的方法、原理和相关的重要科学概念。希望通过本章內容的学习，能够帮助读者充分了解和掌握包括飞秒激光的主要特点、产生方法、传输特性方面的一些关键性的基础知识。
第一节激光脉冲与超短脉冲激光简介
本节主要介绍什么是激光超短脉冲、超短脉冲激光的特点、产生超短脉冲激光的主要方法和超短脉冲激光（包括飞秒激光）的一些重要应用。
一、脉冲与激光脉冲
在科学研究中，通常我们用“脉冲”两个字来描述某一物理量发生突然变化的现象，即“脉冲”是相对于“连续”而言的。人们经常遇到的不同形式的脉冲包括声脉冲、电脉冲、光脉冲等。我们将室内的灯打开又关上，就产生了一个光脉冲（实际上，与此同时我们也产生了电脉冲和对应开关嘀嗒声的声脉冲）．我们看到的闪电是自然界产生的光脉冲。照相吋，闪光灯发出的光是一个光脉冲．行驶到交通路口时红、绿、黄交通灯的交替亮灭也都对应光脉冲的产生。因此，人类生活中实际上充满了各种各样脉冲形式的光。只是这些光脉冲（或称之为脉冲光）的强弱、持续时间乃至颜色等重要特性都有所不同。在现实当中，绝对连续不变的光波是不存在的。从这一点出发我们不妨认为连续光是延续时间接近于无限长的光脉冲。
所谓激光脉冲，顾名思义是指脉冲形式的激光。如果在空间中某处对其进行观测，会发现它仅在短暂的时间内存在，这也正是脉冲的含义。对于单一的脉冲，我们可以用脉冲宽度（或简称脉宽）、脉冲形状、脉冲幅度来对其进行描述，而对多个激光脉冲（可以是接连不断的脉冲，或简称连续脉冲），其基本描述量还包括脉冲重复频率、脉冲间隔的相对抖动、脉冲幅度起伏等。

如果说科学技术进步89~命内涵在于创新，屈光手术领域则始终享受着创新带来的成果，同时，也在诠释着创新在专业发展中的推动作用。激光应用于眼科屈光手术领域整整3。年．纵观近代角膜屈光手术发展史，不难发现其始终处于变迁、革新之中。手术方式从准分子激光最早成功应用于角膜屈光手术的PRK手术，到后来广为流传的LASIK手术：手术部位历经了从角膜表面切削（PRK，tPRK）、表层手术（LASEK、Epi—LASIK）到板层手术（LASIK），而后，浅表板层6，9（SBK）手术，乃至又回归表层以及集表层与板层手术优势共存的各种手术方式。重要的是，新技术与经典术式的融合亦催生了新的手术方式，FS—LASIK、FLEx等技术是飞秒激光技术与LASIK相结合的产物。而更加令人欣喜的是，飞秒激光的应用成就了手术设计理念的重大突破，SMILE等手术则是集各手术优势较具代表性的创新成果之一。在手术形式上，从角膜切削术到植入物，再到组织的成型及透镜的取出，更是千差万别、种类繁多。科学技术的不断发展为我们提供了各式各样8．9方法和手段，新的技术、新的理念层出不穷，面对众多8．9手术方式，它们的特点如何？我们将如何选择？本书尽可能予以展现。

有人说，在激光没有出现之前，人类历史上从未有过这样安全有效的屈光矫正手术。无论其准确与否，至少说明激光矫正方式（0aser．visio门COl“recl：i。n，LVC）为屈光不正的矫正开辟了全新的领域，做出了全新的尝试。我们这些屈光手术医师（被命名为第一代），与更年轻的医师们（第二代、第三代）一起，在不断尝试新的技术的过程中，不仅体验到这些新技术的精准、快捷与有效，同时也经历了许多前人未曾涉及的困惑，面临着这些新技术对我们所提出的新的挑战。
飞秒激光目前应用较普及，也是未来发展的方向，但是迄今为止，国内尚无一部飞秒激光专著，值此出版之际，恰缝全飞秒SMILE手术全国培训中心已接待来自全国各地医院的近、00名医生来中心交流学习。他们最多的反映是无较系统的书籍可参考。也是本书的编写动机之一。因此，本书以飞秒激光为主线。但事实上，飞秒激光仅作为一种工具和手段，是一媒介，本书真正的内容仍是围绕角膜屈光手术展开。如各类手术的治疗原理、适应证、并发症及其处理等。
与这一领域的患者常有较高的期望值一样，我们也不满足于简单的视力提高，更希望在术后不仅获得更好的视功能，而且能够尽量避免副作用和损害：希望在矫正目的达到的同时，尽可能保持组织结构的完整。新目标对我们提出了更高的要求。视觉光学的进步彻底改变了我们对视光的认识，而医学的发展、新型检查技术和手段时出现也需要我们对人眼超微结构及各类损害的表现有重新的认识，需要我们以科学、严谨的态度，对各类新的治疗技术（如激光）对人眼组织的影。向及其规律有更深入的理解与掌握。
