第二节健康教育的基础
健康教育是区别于其他部门的一个独立的医学专业技术领域。健康教育工作的特定目标是改善人们的健康相关行为。过去历史上不曾有过任何一个临床医学或预防医学的专业部门以此为特定目标。健康教育是医学科学与行为科学相结合的产物，因此构成健康教育学科的专业理论基础包括医学（尤其是预防医学）知识和构成行为科学的学科（心理学、社会学、文化人类学，等）知识。
健康教育特有的专业技术方法是流行病学、传播学、教育学、行为医学和管理学等领域的技术方法的综合。健康教育具体工作的一般程序包括健康教育诊断、健康教育干预和干预效果评价，类似于临床工作诊断、治疗、效果评价三大步骤。尽管健康教育的理论和方法目前还不够成熟，但无疑有广阔的发展前景。
健康教育的成败在于使受教育者形成接受教育的态度，建立健康信念，激励健康动机，纠正不良行为，采纳健康行为，实现“知（识）一信（念）一行（为）”的改变。实现这一过程的基础是流行病学、医学社会学及行为科学。要求健康教育人员掌握“健康教育学”、“健康行为学”、“健康传播学”、“健康心理学”、具备电化教育手段方面的知识和卫生美术鉴赏能力等一系列知识和技能。
1．流行病学和医学社会学流行病学是研究疾病分布规律及影响因素，借以探讨病因，阐明流行规律，制订预防、控制和消灭疾病的对策和措施的科学。医学社会学是运用社会性的理论和方法，研究医疗领域中的社会角色、角色关系、角色行为、角色流动、医疗社会组织的交互作用以及医疗领域与整个社会生活的互动及其变化规律的科学。包括教育对象所在社区中危害人群健康的主要因素、危害程度、分布及作用规律；社区人群的生活质量的影响因素等。
2．行为科学许多疾病与不良生活方式、习惯和行为有关。中国预防医学科学院寄生虫病研究所抽样调查了726个县近150万人，结果经推算全国感染蛔虫的人数达5.31亿，其中绝大多数为中、小学学生。寄生虫感染中混合感染（感染两种或两种以上）占43．33％，最多的人感染九种寄生虫。寄生虫感染的原因，主要是生食淡水鱼、虾、蟹、吃腌制、熏烤、暴晒的肉制品，饮用河浜水；另外，集贸市场家畜、鱼类缺乏食品卫生监督，使寄生虫感染率呈上升趋势。有目的地进行行为习俗的矫正，即可控制某些疾病的发生和流行，用行为科学与健康教育解决人群的某些健康问题势在必行。1987年wHO提出的腹泻病控制规划策略。在五个主要策略中有三个是行为措施，即改善营养、安全用水和良好的个人及家庭卫生习惯。但也应该认识到，行为作为社会现象有其深远的历史根源和社会基础，要影响和改变人们健康的偏离行为是一项十分艰巨的任务。
