Author Description

李平，中日友好医院临床医学研究所副所长，药物药理研究室主任；研究员，主任医师；北京中医药大学兼职教授、博士生导师，中国协和医科大学博士生导师。1982年毕业于北京中医药大学，曾在中国中医科学院获得中医内科临床硕士和博士学位。1992年获得世川医学奖学金，在日本新泻大学医学部进修，并获得该大学医学博士学位。
主要从事中西医结合肾脏病基础与临床研究。现为北京市中西医结合学会肾脏病专业委员会主任委员；全国中西医结合学会肾脏病专业委员会副主任委员；中华中医药学会肾脏病分会副主任委员；世界中医药联合学会临床疗效评价专业委员会副会长。2003年被国家六大部委评为国家留学回国人员先进个人。2007年被评为国家中央机关优秀女科技工作者。2007～2008年度卫生部有突出贡献的中青年专家。
先后作为国家“973”项目、国际合作重点项目、国家自然基金重大研究计划项目等国家及省部级科研课题10余项科研课题负责人。在国内外学术杂志和学术会议上发表论文100余篇，其中SCI收录杂志论文15篇；在国际会议上做特邀报告和专题报告10余次。2001年以来获中国中西医结合学会科技进步一等奖2项、中华中医药学会科技进步二等奖1项、北京市科技进步三等奖1项和中日友好医院科技进步二等奖1项。撰写学术著作9部，其中主编3部，副主编2部，获国家图书特别奖1项、中华中医药学会科技著作二等奖和中华中医药学会科谱著作二等奖各1项。

上篇 基础篇
第一章 糖尿病肾病的基因研究
遗传易感性被认为是糖尿病肾病（diabetic nephropathy，DN）发生的重要原因。Saperstein等最早提出糖尿病（diabetes mellitus，DM）微血管病变是由遗传基因决定的，他们报道糖尿病父母的子女中虽有53％在临床和实验室检查无任何糖代谢紊乱，但肌肉活检已有微血管基底膜增厚。大量资料显示，遗传因素在糖尿病肾病的发生中起关键作用。早在20年前，在胰岛素依赖性糖尿病（insulin-dependent diabetes，IClIM）的前瞻性队列研究中就发现了遗传因素可能在糖尿病肾病的发病机制中起一定作用。后续的研究发现，控制血糖可减少糖尿病肾病的发生，但仍有30％的1型患者糖尿病（type 1 diabetes：mellitus，T1DM）和15％～60％（存在各族差异）的2型糖尿病（type 2 diabetes reellitus，T2DM）患者最终将发生糖尿病肾病。相反，在糖尿病病程大约30年后，糖尿病持续蛋白尿的累积发生率停止增加，即便是在血糖控制不佳的情况下，也仍有50％的患者不发生终末期肾衰竭。这说明血糖的控制不良仅是糖尿病肾病发生所必需但又不具备完全足够的条件。
