Book Abstract

4 中药复方的药理
古之谓“七方”，有大、小、缓、急、奇、偶、复，其中“复方”为二方、三方及数方相合之方，或别加余药及分两均齐之方。其意与今之复方概念有别。本章所论复方，系指两种或两种以上的药物，按照中医的四诊八纲、辨证论治的原则，针对病情有机地组合而成的方剂，系与单味药相对而言。由于疾病的病程和性质复杂多变，往往寒热交错，虚实并见，一时一身而数病相兼，只凭单味药难以照顾全面，故须将多种药物适当配合，利用其相互间的协同或拮抗作用，提高疗效或减少不良反应，以适应复杂病情的治疗。
追溯人类用药的历史，是以用单味药开始的。随着人们对药物认识的不断深化和对病因病机理解的逐步提高，才逐渐将药物配伍使用。《五十二病方》就是在这种思想指导下发展起来的。张仲景的伤寒金匮方，便是急慢性传染病学和多科杂症治疗的典范，是中国医学高度光辉发展的里程碑。从单方过渡到复方，是中药治疗学上的一个飞跃。
4·1 配伍原则
中药配伍是在长期医疗实践中积累的丰富经验和知识。《本草经》记载：“药有单行者，有相须者，有相使者，有相畏者，有相恶者，有相反者，有相杀者。凡此七情，合和时视之，相须相使者良，勿用相恶相反者。”说明药物有协同作用（相须相使），拮抗作用（相畏、相恶、相杀），相反作用（十八反）等，在配伍制方时均应注意。
4·1·1 相须相使配伍
李时珍说：“药有七情：独行者，单方不用辅也。相须者，同类不可离也，如人参、甘草、黄柏、知母之类。相使者，我之佐使也。”相须即是性能类似、药效方向一致的药同用，使其发生协同作用，提高疗效。荆芥配防风，同属辛温，可加强散风解毒作用；黄连、黄芩与大黄，同属苦寒，配伍后其清热解毒、降压作用加强；桂枝与麻黄配伍，可加强后者发汗解热功效。大黄配芒硝、泻下作用增强；地丁与蒲公英，消肿解毒作用增强。实验也证明黄连解毒汤较单味黄连抗菌作用强，并可延缓细菌耐药性的产生。
复方除具有上述增效作用外，还显示了多效性。遇病情复杂，一味药不能奏效时，可发挥复方的多效性，采取标本兼顾，照顾各个证候，全面施治。以普济消毒饮为例，方中芩、连、连翘、板蓝根清热解毒；薄荷、牛蒡子、僵蚕、柴胡疏散风热；桔梗、玄参、马勃能清热利咽；甘草、陈皮以和胃调中。本方为治大头瘟高热的主方，充分体现了治疗瘟毒的头面、咽喉诸证的作用。又如紫雪丹一方，其中生石膏、寒水石及滑石甘寒清热；玄参、升麻、甘草、犀角清热解毒；羚羊清肝热熄风；朱砂、磁石安神镇痉；麝香、木香、丁香、沉香以行气开窍；朴硝、硝石以泄热散结，所以治疗高热神昏等证有多效。两方全方虽尚缺少现代实验研究，但方中各药大多经过研究，表明有很好的抗菌、抗病毒、解热、镇静及抗惊作用。
多效并不等于万能，也非一方统治万病。清·吴鞠通说：“无不偏之药，则无统治之方……”所以方剂都有其针对性，并设加减法，显示其灵活性。
