Book Abstract

【不良反应】

注射部位可出现疼痛、局部红肿，也可出现轻度发热，均可自行缓解。罕见局部无菌性化脓、过敏性皮疹、休克、紫癜、血管神经性水肿、变态反应性神经炎等。

（六）皮上划痕用鼠疫活疫苗

【预防的疾病】

鼠疫。

【种类】

鼠疫弱毒株活菌体制成的疫苗。

【接种对象】

用于疫区或经过疫区的2～60岁人群。

【免疫程序】

每年接种1次，遇有疫情，疫区人员在第一次接种后6个月再种一次。疫苗接种后10天开始产生免疫力，1个月抗体水平达高峰，6个月后开始下降，1年后免疫力降至最低，故有感染危险者应每年接种1或2次。

【免疫持久性】

1个月抗体水平达高峰，6个月后开始下降。

【禁忌】

对已知疫苗成分过敏，患急性疾病、严重慢性疾病、慢性疾病的急性发作期和发热者，免疫缺陷、免疫功能低下或正在接受免疫抑制剂治疗者，妊娠期或6个月内的哺乳期妇女等。

【注意事项】

皮上划痕用，严禁注射。

【不良反应】

一般无全身反应，偶有微热、不适、局部淋巴结轻度肿大，可自行消退，不需任何处理。皮上划痕用鼠疫疫苗误用于注射，则可引起严重临床反应，局部严重红肿，甚至无菌化脓、溃疡。全身反应有高热、局部淋巴结肿痛、头痛、不适。

（七）乙肝疫苗

【预防的疾病】

用于预防乙型病毒性肝炎（乙肝）。

【种类】

目前我国有3种重组乙型肝炎疫苗，分别为重组乙型肝炎疫苗（酿酒酵母）、重组乙型肝炎疫苗（汉逊酵母）和重组乙型肝炎疫苗（CHO，中华仓鼠卵巢细胞）。

【接种对象】

新生儿，特别是母亲HBsAg阳性者；所有乙肝易感者（乙肝各项感染指标均为阴性者）。

【免疫程序】

全程接种3剂，接种时间为O、1、6个月，即接种第1剂后，1个月后接种第2剂，6个月后接种第3剂。

【免疫持久性】

研究资料显示，接种乙肝疫苗后保护效果可以达到近20年。从临床的人群长期免疫效果来看，全球还没有一个国家对接种人群实施加强接种，所以目前我国尚未制定乙肝疫苗加强接种的免疫策略。

【禁忌】

已知对该疫苗的任何成分，包括辅料以及甲醛过敏者；患急性疾病、严重慢性疾病、慢性疾病的急性发作期和发热者；孕妇；患未控制的癫痫和其他进行性神经系统疾病者。

【注意事项】

安瓿破裂、有摇不散的块状物时不得使用；应备有肾上腺素，以防偶有过敏反应发生时使用。

【不良反应】

一般情况下，接种乙肝疫苗后不会影响日常工作和生活，常见的接种反应为少部分接种者出现注射局部的反应，如红、肿和疼痛。

（八）甲肝疫苗

【预防的疾病】

用于预防甲型病毒性肝炎（甲肝）。

【种类】

目前有两种：甲肝减毒活疫苗和甲肝灭活疫苗。

【接种对象】

≥18月龄人群。
