治疗总体指导方针
■矫形按摩一般持续1小时，具体每节时间长短可以变化很大。在开始调整之前按脊师已经成功地运用了每节15分钟的按摩。按摩师应用按摩每节30分钟。
■大多数情况下按摩每周1次，持续4-6周。症状比较急的患者，按摩频率可以更频繁些。慢性疼痛的患者可以每周1次，按摩数月；然后逐渐过渡到每月2次，再维持几个月；然后每月1次。依此类推直到达到最好的疗效（MMI）。按摩的频率经常依赖于患者能否保持所取得的功能进展。OM和MET对保持慢性病例的功能进展很有帮助。
■每进行6次按摩就对患者做一次评估，决定是否需要继续治疗。多次评估的目的是确定治疗取得的主观和客观进展，为进一步治疗收集客观检查结果。如果只有很小进展或没有进展，需要评估是否有必要转诊。
■矫形按摩也能用于增进健康（健身）、体育运动、舞蹈和娱乐等方面。
急性病例的按摩计划
■要确定按摩计划首先要清楚地了解患者病情的严重程度。通过询问病史，排除危险信号，确定该患者适合接受按摩治疗后，可以开始评估。
■对脊柱损伤或功能障碍的病例，从身体正常的一侧开始按摩，这样不仅对患者更容易，而且可以将放松的反应从一侧传到另一侧。
■如前所述，告诉患者在接受按摩和MET时应该完全放松。
■按摩脊柱时，以全身轻柔的晃动开始，就像第3章中提到的第一类按摩手法那样。这样能以非侵入性的方式接触患者，并确定患者的防备程度。如果患者的身体对抗晃动，要减慢速度，直到感觉患者放松。可以轻声告诉患者“让我晃动你的身体，这种晃动就像摇晃小孩睡觉一样，轻放松”。

■记住疼痛的消退并不代表正常功能的恢复。一旦疼痛消失，需要再次评估该部位，以确定关节活动范围正常，该部位肌肉的长度和力量处于平衡状态。
慢性疼痛患者的按摩计划
■大多数慢性疼痛患者的按摩均包括脊柱按摩，即对腰骶部、胸椎和颈椎做第一种按摩手法，然后将按摩的重点放在主诉部位。
■由于需要松解软组织的粘连，按摩的手法通常较深。按摩应是患者不舒服的部位，让患者尽量保持完全放松，即使可能有不适感。否则提醒患者在做不到完全放松时要随时提出。
■主要采用MET，或采用按抚手法。
■按摩纤维化的部位时需要大幅度地晃动患者，按摩不稳定的部位时只能做小幅度的运动。
■告诉患者正确的姿势。不良姿势带来的应力会加重慢性紧张模式，而这可能正是患者疼痛的原因。
■适当的运动指导对慢性疼痛患者十分重要，包括力量、稳定、协调和平衡训练。
