第一章肺功能检查：回顾与展望
呼吸生理功能研究的迅速进展是近代医学科学的重要成就之一。疾病的诊断从病理诊断、病因诊断进一步深入至机体器官、组织的功能诊断，而且更趋完善。胸部X线影像、电子计算机断层扫描（CT）、呼吸组织病理及免疫组化等检查反映的是静态组织学改变，而呼吸生理却是研究活体的动态功能学改变。生理功能的维护与修复是疾病诊治的重要环节。肺功能检查是运用呼吸生理知识和现代检查技术来了解和探索人体呼吸系统功能状态的检查，是临床上胸、肺疾病诊断、严重度评估、治疗和预后评估的重要检查内容，广泛应用于呼吸内科、外科、麻醉科、儿科、流行病学、潜水及航天医学等领域。
由于肺功能检查的内容和方法众多，限于篇幅，本章简略回顾了肺功能研究和临床应用的发展历程，并展望了肺功能今后的发展方向，以冀帮助读者对肺功能检查有一个初步的了解。
一、肺功能检查技术的发展
肺功能检查与临床研究已有数百年的历史。从最初的肺容量检查逐渐发展至呼吸流量检查、肺内气体交换检查、呼吸动力学检查、运动心肺功能检查等多种检查项目和方法，检查技术也不断推陈出新。

Introduction

肺功能检查是临床上胸、肺疾病及呼吸生理的重要检查内容，广泛应用于呼吸内科、外科、儿科、麻醉科、流行病学、运动医学、潜水及航天医学等。对于早期检查出肺和气道病变、诊断气道病变的部位、鉴别呼吸困难的原因、评估疾病的病情严重程度及预后、评定药物或其他治疗方法的疗效、评估胸肺手术的耐受力、评估劳动强度及耐受力以及对危重患者的监护等，肺功能检查均有重要的临床指导意义。近年中华医学会呼吸病学分会发布的“慢性阻塞性肺疾病诊治指南”、“支气管哮喘防治指南”、“咳嗽诊断与治疗指南”等疾病的诊治指南中，均将肺功能作为这些疾病诊断和严重度分级的重要指标，甚至是金标准。
目前肺功能检查已在我国大中型医院普遍开展。在医院等级评定的标准中，卫生部要求二等甲级以上的医院均应开展肺功能检查。随着我国社区医疗工作受到越来越多的重视，基层及社区医院的肺功能检查也必然开展得越来越广泛。卫生部已将肺功能检查列入“面向农村和城市社区适宜卫生技术推广十年百项计划”进行推广，但笔者近年曾对我国肺功能临床应用的现状进行了调查，发现我国肺功能的开展极不平衡，特别在中小型医院肺功能的临床应用很多处于空白状态，即使在部分大型教学医院，其肺功能检查的质量控制也亟待提高。
