第一章卒中重症加强医疗
单元概况
卒中重症加强医疗单元(stroke crilical care unit，SCCU)是对急性卒中患者提供专业加强护理和治疗的病房，这个病房不但要提供专业的神经系统的监护和治疗以及预测神经功能预后和判断脑死亡，同时要兼顾全身各个脏器功能的监护和维护。
一、卒中重症加强医疗单元的组织
急性脑血管病的重症患者的神经功能预后随着对心肺功能的治疗技术改进而得到改善，与此同时还有许多专门针对神经功能的治疗手段．如动脉溶栓和神经保护药物等。而静脉或动脉溶栓治疗又有可能增加。出血转换的风险．所以适当的血压控制及由专业人员的密切监护可以降低出血的发生，同样脑出血和蛛网膜下腔出血患者是否需要手术治疗也需要由专门的医师来决策。故SCCu应该有独特的设计、组织及文施的方法。

2003年3月美国专科医生委员会（ABMS）批准美国精神病学和神经病学委员会（ABPN）关于设立血管神经病学资格考试的申请。从此一个新的学科正式产生，这就是血管神经病学（vas—cularneurology）。血管神经病学是特定的医学领域。它利用临床检查、影像、介入技术、药物对中枢神经系统缺血和出血性疾病进行评价、监护、治疗和预防。血管神经病学医师参与血管病的多学科医疗，包括流行病学、基础科学、临床神经病学、神经影像、重症监护、血管内介入治疗、神经血管外科、神经外科、神经超声、脑血流与代谢、神经行为、神经康复。同时血管神经病学医师获得中枢神经系统血管支配及其病变时改变的详尽知识，并且管理门诊、病房、监护室的卒中患者。血管神经病学的产生把脑血管病的临床推向一个全新的阶段。
2004年。我和姜卫剑教授、高培毅教授在首都医科大学为研究生开设了《血管神经病学》的课程，反响之好，超出了我的想象。之后。结合血管神经病学的基本理论。
