Author Description

邱贵兴，院士，1968年毕业于中国协和医科大学，现任北京协和医院外科学系主任、骨科主任，主任医师、教授、博士生导师，全国政协委员，中华医学会骨科学分会主任委员，中华医学会第23届理事会常务理事，北京医学会第17届理事会常务理事，中华骨科杂志、中华关节外科杂志、中国骨与关节外科杂志主编，国际脊柱畸形矫形研究组（SDSG）中国部主席、国际脊柱功能重建学会（SAS）中国分会主任委员等职。主编、主译《骨科手术学》、《骨科学》等30余部专著。在国内外各种杂志发表论著400余篇，曾荣获国家科学技术二等奖2项等多个奖项，并多次荣获中国科协先进个人、北京市总工会教育创新标兵、北京市教育工会师德先进个人等个人荣誉。为北京协和医学院教学名师、优秀教授、优秀研究生导师。

Book Abstract

一、无菌原则
创口的愈合有两个基本条件：一是丰富的血供，二是创口的清洁。手术的无菌原则是创口清洁的重要保证。
外科手术的基本步骤包括皮肤的切开，深部组织的分离，病变部位的切除修复等操作。皮肤是人体的屏障结构，皮肤一旦被切开，深部组织即和外界接触，此时，如果没有严格的无菌操作，环境中的致病微生物即会进人体内，在疾病状态下，病人机体抵抗力下降，进人人体的致病微生物一旦不能被机体有效清除，就会引起术后创口感染的发生。
创口感染不但会影响创口愈合，导致瘢痕增生，加重局部粘连，严重者还会危及病人的生命。无菌原则是手术成功的保障，也是病人生命健康的保障。外科手术必须严格遵守无菌原则。
二、微创原则
丰富的血供，健康的组织是切口局部顺利愈合的前提，而要满足这两个条件，手术中就必须坚持微创原则。
微创原则就是手术中以轻柔稳健的操作减少组织的额外损伤。在创口局部保留尽可能多的健康成活组织，以利于手术创口的恢复，减少创口瘢痕的形成。微创包括以下两个方面的内容。
1．微创的概念微创的概念要求手术操作者将微创贯彻到手术的全部过程，具体到手术的每一个细节，微创和微小切口是两个不同的概念，小的切口只是使术后瘢痕变小，可达到一定的美容效果，但如果深部操作粗糙，同样不能算是微创手术。
2．微创器械为了达到微创操作的要求，术者应尽量选用能够最大限度减少组织损伤的器械，如切开时用尽可能锋利的切割器械，缝合时根据不同的情况选用合适的缝合材料等。
