第一章 绪论
医院感染是指住院病人在医院内获得的感染，包括在住院期间发生的感染和在医院内获得出院后发生的感染；但不包括人院前已开始或人院时已处于潜伏期的感染。它的研究对象不仅包括住院病人、门诊病人、急诊病人、陪护人员、探视人员，还包括医务人员。
医院感染虽然存在已久，过去对之研究采用的是适用于传染病研究的理论和方法。但近年来，随着时代的发展、医学的进步，各种新的诊疗操作新技术的开展、免疫抑制剂及广谱抗生素的应用，以往不致病的条件致病菌导致的内源性感染增加，加之各种因素的影响，医院内各种感染性疾病的出现已不能单用传染病的观点解释，人们才逐步认识到医院感染学科的必要性，必须采取新的理论和特殊的方法进行研究。
第一节 医院感染研究现状
一、医院感染研究的起源与发展
医院感染与医院相随并存，并随着医院的发展而日显复杂。对医院感染的研究经历了由简单到复杂，从单纯消毒隔离到病因学、流行病学、病理生理学、诊断学、免疫学、消毒学、管理学、治疗学等复杂的研究过程。
我国明朝的李时珍（1518～1593）曾在本草纲目中记载：蒸过的衣服不会传染疾病，提示有些疾病可以互相传播，高温可以去除某些感染因子。俄国的外科医师皮罗果夫（1810—1881）从多年实践中大胆断定，大部分病人不是原发病本身，而是由于医院感染而死亡。英国的南丁格尔（1820—1910）为减少伤口感染采取了病房通风、隔离病人的方法，并提出医务人员戴手套是预防交叉感染的有效方法。法国的巴斯德（1822—1895）在显微镜下发现了空气中的微生物，l867年他提出手术时用苯酚喷雾消毒空气。英国的李斯特（1827—1912）首先阐明了细菌与感染的关系，在巴斯德的基础上把消毒范围扩大到医务人员手、器械、敷料，从而大大地降低了伤口感染率。1928年美国微生物学家弗莱明发现了青霉素，并于40年代由美国研制成功，投入临床使用，从此进入抗生素时代。半个多世纪以来，人类不断地研究生产各种抗生素，而微生物（主要为细菌）也不断对抗生素产生耐药。目前多重耐药菌株感染治疗非常困难，而且通过质粒介导进行广泛传播。多重耐药菌感染和流行给人类造成新的威胁。
