Book Abstract

各国情况不同，监测系统各不相同。我国ADR监测报告工作由国家食品药品监督管理局主管。监测报告系统由国家ADR监测中心和专家咨询委员会、省市级中心监测报告单位组成。
（一）药物不良反应报告制度及要求
《药品不良反应报告与监测管理办法》明确指出我国ADR实行逐级、定期报告制度，必要时可以越级报告。新的或严重的ADR应于发现之日起15日内报告，死亡病例须及时报告。
药品生产、经营、使用的单位和个人发现可疑的药物不良反应病例时，需进行详细记录、调查，按要求填写报表，并向辖区药物不良反应监测中心报告。
（二）药物不良反应报告范围
我国规定ADR的报告范围：上市5年以内的药品和列为国家重点监测的药品，报告有可能引起的所有可疑不良反应；上市5年以上的药品，主要报告严重的、罕见或新的不良反应。严重的药物不良反应是指因服用药品引起以下损害情形之一的反应：①引起死亡。②致癌、致畸、致出生缺陷。③对生命有危险并能够导致人体永久的或显著的伤残。④对器官功能产生永久损伤。⑤导致住院或住院时间延长。
新的ADR是指药品使用说明书或有关文献资料上未收载的不良反应。对上市5年以上的药品，对已知的比较轻微的不良反应不要求报告，如三环类抗抑郁药引起的口干，阿片类所致便秘，地高辛引起的恶心等。

自20世纪90年代以来，有关口腔医学的专著、参考书籍犹如雨后春笋，数量剧增。书籍编撰的风格各有不同。有的堪称上乘之作，但重复雷同，涉嫌因袭者亦可见到。为此，上海世界图书出版公司要组织出版一些口腔医学参考书时，我们不由得有点心中犯难，就怕写出来的东西又成了重复的陈货。经过一番思考和讨论终于确定了本丛书编写的指导原则，即以专题为主；以临床口腔医学为主；以国内外医学的新成就、新经验为主；并力图打破原来的学科界限和体系来组织编写一批高级口腔医学参考书
口腔医学是医学中的一级学科。按照多年来的习惯，在临床口腔医学中又可分为若干个亚科，诸如口腔颌面外科学、口腔内科学、口腔正畸学、口腔修复学等等。其中有的与国外相同，如口腔颌面外科学；有的则不尽相同，例如口腔内科学。当代最具创新或创造性的成果都是产生于各学科或多门学科的相互交叉点或切点上，生命科学出现了学科间交叉、整合、重组的趋势。科学研究如此，临床医学亦莫不如此。学科的整合在基础医学方面当为在分子水平上的整合，例如“分子医学”的崛起；在其他方面则表现为学科与学科之间，科学与技术之间，以及自然科学与人文科学之间，生命科学与非生命科学之间的整合重组，近年来出现的所谓“Bio-x”中心，即生命科学与非生命科学结合的体现。为此，口腔医学的各个学科之间也面临着这一命题，而且在国外业已有一定的经验可资借鉴。在这一原则的思想指导下，我们也试图适应潮流，学习国外的先进经验，打破传统的学科系统来出版一些重新整合的专著，如《保存牙科学》、《颌面颈部肿瘤影像诊断学》和与旧的“口腔内科学”概念完全不同的《口腔内科学》等，以适应新形势的需要.
