第1章疼痛的发生机制及镇痛药物作用原理

第一节疼痛的分类与发生机制概述

世界卫生组织（World Health Organization，WHO，1979）为疼痛所下的定义是“疼痛是组织损伤或潜在损伤所引起的不愉快感觉和情感体验”。根据疼痛的持续时间以及损伤组织的可能愈合时间可以将疼痛分为急性疼痛和慢性疼痛。

一、急性疼痛（生理性疼痛）

急性疼痛相对通用的定义为：与组织损伤、炎症或疾病过程相关的，持续时间相对较短（通常短于3个月）的疼痛类型。急性疼痛通常是组织损伤的标志，促使个体采取适应性或保护性的行为，因而在大多数情况下对机体具有保护作用，此外急性疼痛中的组织损伤和疼痛体验往往存在一一对应关系，随着损伤组织的痊愈，疼痛也随之消失。急性疼痛为伤害感受性疼痛，伤害感受性疼痛的发生是疼痛形成的神经传导基本过程。疼痛形成的神经传导基本过程可为4个环节。伤害感受器的痛觉传感（transduction），一级传人纤维、脊髓背角、脊髓一丘脑束等上行束的痛觉传递（transmission），皮层和边缘系统的痛觉整合（interpretation），下行控制和神经介质的痛觉调控（modulation）。理论上，阻断任何环节都可以使疼痛缓解。

二、慢性疼痛（病理性疼痛）

与急性疼痛相对应，慢性疼痛被定义为组织损伤痊愈后依然持续存在的、或者持续时间超过3～6个月的一种疼痛类型。慢性疼痛有炎性疼痛、神经病理性疼痛和两者的混合类型。神经病理性疼痛的定义为“来自外周或中枢神经系统的病变或功能紊乱所引起的疼痛”。
