Book Abstract

2-258题解：超声雾化吸入适用于痰液黏稠不易咳出者，但哮喘病人不宜使用，因喷雾颗粒过小，较多小雾点进入肺泡或过饱和的雾液进入支气管可作为异物刺激，使支气管痉挛导致哮喘加重。
2-266题解：慢性肺源性心脏病病因以支气管、肺疾病多见，引起肺动脉高压，长期肺循环阻力增加，右心负担加重，右心室代偿性肥厚。随着病情发展，心脏储备能力逐渐减退，缺氧又使心肌损害，当呼吸道发生感染，使缺氧加重或由于其他原因使肺动脉压进一步增高超过右心室的负荷时，右心室即行扩张，最后导致右心衰竭。根据病人的临床表现与实验室检查可以判断发生了慢性肺源性心脏病。
2-272题解：本病例根据慢性咳嗽、大量脓痰、反复咯血和继发肺部感染等临床特点，应首先考虑诊断为支气管扩张，而确诊此病最可靠的依据则是做支气管碘油造影。
2-303题解：用于呼吸衰竭的兴奋剂以尼可刹米（可拉明）为主，它可直接兴奋延髓呼吸中枢，也可通过刺激颈动脉窦和主动脉体的化学感受器，而反射性兴奋呼吸中枢，当呼吸处于抑制状态时，兴奋作用较明显（虽然作用温和，但比较安全），但一次用药仅维持5～10分钟，故常静脉间歇给药，大剂量时可出现心律失常、恶心、呕吐、震颤等。故本题答案是呼吸兴奋剂过量。
2-355题解：病人有烦躁不安、呼吸频率及心率均加快，且有球结膜充血均提示呼吸衰竭，此时既不能降低吸氧的浓度，更不能加大氧流量和使用镇静剂，因不是声门水肿堵塞，故气管切开无必要。应给予呼吸中枢兴奋剂。
2-448题解：慢性肺源性心脏病病人因有缺氧和二氧化碳潴留，一般应给予低流量、低浓度持续吸氧；饮食宜选用高热量、高蛋白、低盐、易消化的食物；急性发作期应卧床休息，以减轻心脏的负担。所以，高流量、高浓度持续给氧，高盐饮食及急性发作期体育锻炼是错误的。而鼓励咳嗽及时清除痰液；抬高下肢以减轻或消除水肿是正确的。
