Author Description

张抒扬，北京协和医院心内科主任医师、教授，北京协和医院临床药理研究中心主任、心内科副主任。师从我国著名的心血管病专家方圻、金兰教授，1991年在中国协和医科大学获医学博士学位。在北京协和医院经历严格的内科住院医师、内科总住院医师以及心脏主治医师的训练，1995～1999年在美国进行博士后研究。多年来一直从事心血管临床的医疗、教学和科研工作，是一位深受患者信赖和称赞的好医生。
吴炜，北京协和医院心内科主治医师。1996～2004年就读于中国协和医科大学，获临床医学博士学位，期间两次获得中国协和医科大学医学生最高荣誉——“协和之友”称号。2007年12月至2008年2月以交换住院医师身份赴美国加州大学旧金山分校交流访问，期间获得优秀住院医师、优秀总住院医师称号。曾担任多项国际性医学会议同声传译，并参与编译《Topol心血管病学》（第3版）。参与国家自然科学基金1项，承担北京协和医院青年科研基金1项。在国内核心期刊杂志发表文章10余篇。

Book Abstract

为了分担大小两套血液循环，心脏分成左右两半，左半就叫左心，是大循环的驱动泵，右半就叫右心，分担小循环的任务。左心将血液通过体动脉送到头部、躯干、腹内脏器和四肢，带去各种营养、必需的物质和充分含氧的红细胞，供应它们的不同需要，使之活动完好，完成各自的任务。通过组织器官，静脉内血液又将剩余的物质和二氧化碳带到右半心脏，右心将血泵入肺，在肺内血液与进入肺泡的空气进行气体交换，排出二氧化碳，吸进氧气，使血液重新获得充分的氧气后又通过肺静脉回到左心，开始第二轮周游全身的供给工作。这就是一个循环过程。这个循环过程是通过心脏的收缩和舒张（这就是我们可以感觉到的“心跳”）来完成的。
在心脏解剖上，包含有两套系统，静脉血流系统和动脉血流系统。静脉血流系统要把体内各器官所流回的血液收集到上腔静脉及下腔静脉，静脉血流回来之后，继续汇集到右心房，再经三尖瓣到右心室，右心室收缩时将血经肺动脉瓣把血泵人到肺动脉，到达肺脏。这些静脉系统的血流经肺循环气体交换以后，再由肺静脉回流，含氧量高的血再聚集于左心房。当左心房收缩和左心室舒张时，压力差异会冲开二尖瓣，使得血液进入左心室。左心室强力收缩则能打开主动脉瓣，将血液泵人大动脉进而输送至全身，从而完成体循环。
左、右心房，左、右心室两两之间有隔间存在，分别称为房间隔及室间隔。比如，大家可能听说过的先天性心脏病——房间隔缺损（房缺）和室间隔缺损（室缺），就是房间隔或室间隔在胎儿发育过程中出现了问题，导致左右心房或左右心室之间出现了血流相通。本来左右心的血液的氧气和营养物质的含量是不相同的，而且左右心的压力也是不同的，但是这种房间隔或室间隔的缺损使得左右心之间的血液相混合，该走体循环的血液去了肺循环，会产生不良后果。想象一下，本来应该走京沪高速公路的货车结果上了京哈高速公路，该去上海的货物运到了哈尔滨，怎么能不乱了套呢？
