第一篇微生物感染
第一节呼吸道病毒感染
一、普通感冒
普通感冒是大多由呼吸道病毒引起的经飞沫传播的上呼吸道疾病，临床上可有鼻塞、流涕，咽痛，轻微咳嗽，少量咳痰，偶有低热。常为自限性。
【治疗】
无需特殊抗病毒治疗，注意支持、对症治疗。
二、流行性感冒和副流行性感冒病毒感染（高致病性禽流感）
流行性感冒（简称流感）是由流感病毒引起的、经飞沫传播的急性呼吸道传染病，临床上有急起畏寒、高热、头痛、乏力、全身肌肉酸痛和轻度呼吸道症状。发病率高，传染性强，但病程短。一般为3～4日，常呈自限性。婴幼儿、老年人、免疫功能地低下者和伴有慢性呼吸道疾病或心脏病患者易并发肺炎，且有导致死亡的可能。流感分甲、乙、丙三型，其中甲型流感常有流行发生，威胁性最大。
【治疗】
以下两种药物仅对甲型流感病毒有效：①金刚烷胺：成人200mg Bid.pO.；小儿 4～5mg／（kg·d），分2次口服，老人减半，疗程3～5日；②金刚乙胺：成人100mg，po.Bid.。儿童可用5mg／（kg·d）。
以下两种药物对甲、乙型流感病毒均有效：①扎那米韦：需使用“Diskhaler”专用吸入器经口吸人，成人和>12岁儿童，每次2吸，每吸约5rag（10mg／次），2次／日，连用5日；②奥司他韦：成人75mg／次。2次／日，连用5日。
利巴韦林（病毒唑，三氮唑核苷）可采用2～5mg／ml（生理盐水）滴鼻或20mg／ml雾化吸人，每日3～4次，并可同时口含利巴韦林2～4mg，每2～3小时一次，连续2～3日。重者给予10～15mg／（kg·d），分两次静脉滴注。
副流感病毒主要引起婴幼儿和儿童上呼吸道感染和严重的下呼吸道感染的重要病原。目前已有1、2、3、4四个血清型。1、2型易发病年龄在2～6岁，夏秋季发病较明显，并有隔年1次流行趋势，易引起普通感冒和喉气管支气管炎。3型多见于1岁以内婴儿，以冬末、初春流行为主，引起毛细支气管炎和肺炎。4型病毒因培养困难，目前临床和流行特点尚不够明确。
