第7章角膜接触镜的塑形效果
一、国内临床研究
如何能使患者持之以恒地利用RGPCL保持良好的视觉效果，从而维持正常的生活、学习与工作，根据我们的实际经验提出以下3点建议：①初诊时要与患者及其家属进行很好的交流，建立相互信任的感情。并通过裂隙灯、角膜地形图、角膜厚度、角膜内皮细胞、泪液膜质量、视觉质量等眼部全面检查，明确圆锥角膜的诊断和分级，评价双眼的发病进展情况，需要向患者详细交代病情、详细说明利用RGPCL矫正与控制圆锥角膜的优势所在、在治疗中可能出现的问题，以及各方面注意事项等事宜，使患者及其家属能充分了解和正确面对自己的眼病。②材料和设计的选择与验配要适宜，应充分考虑到患者戴镜的清晰度、舒适度和长期使用的安全性能，只有通过较长时间的试戴，达到医患双方的基本满意之后才可处方RGPCL。③不断教育患者提高依从性，密切配合医师进行定期复查，随时根据眼部的变化和镜片的变化进行治疗和调整，镜片使用周期一般不超过1.5年，按规范程序始终严格清洁、保存镜片，并使患者养成每日自检的良好习惯，一旦发现异常能及时摘镜就诊。实际经验证明长期配戴RGPCL是可行的，在医师高度责任心和耐心的基础上，绝大多数患者均能与医师形成良性互动，长期依赖RGPCL良好的视觉效果从而顺利地投入到正常的学习、工作与生活之中。

Introduction

圆锥角膜顾名思义，其特征是角膜失去正常的弧形，呈现圆锥形或类圆锥形的异常突出变形，从而导致角膜的光学性能严重降低，犹如照相机的镜头发生严重扭曲变形。圆锥角膜是青少年时期较常见的屈光性疑难疾病，好发于15～25岁年龄之间，国内外调查显示其患病率平均为1／2000左右。青少年时期正处于人生的快速生长、发育和开阔视野的关键时期，因此能及早发现本病对减轻青少年视觉发育的危害性尤为重要。绝大多数圆锥角膜患者常波及双眼，虽然并不一定同时发病或程度同等。因此当发现一只眼出现圆锥角膜病变时，同时提醒医师和患者需要高度警惕另一眼的发病，需要采取相应的干预措施，预防和控制本病的进一步发生和发展。借助眼视光领域中角膜接触镜的先进技术可以使圆锥角膜的视觉质量迅速发生质的转变，能恢复或提高至正常视力及双眼视觉功能，从而保障了患者的正常学习、工作与生活，很多患者可以终生受益。随着角膜屈光性手术的增多，医源性圆锥角膜的发生率逐年增加，这提示我们需更加谨慎、科学地选择手术方法，一旦发生术后继发圆锥角膜亦需及时进行屈光重建的矫正与治疗，避免延误。
