Book Abstract

晚期并发症疗效欠佳
疗效欠佳是由于耳廓“招风”外观的再现所致。它可发生较早（在术后l周），但也会很晚（数月后）。原因是耳廓软骨的弹性未能通过手术充分消除（图4-15）。遇到这种情况经常需要再次手术。按前述同样的方法，处理软骨时必须尽量完全彻底，行深达对侧面软骨膜的贯穿软骨切开以便完全松解对耳轮、耳轮和耳甲。在成形对耳轮软骨管时，应使用不可吸收缝线固定。如有耳垂前突（图4、15），可用前述的方式，或采用修复的方法来解决。

对耳轮软骨管成形过度与俗称的“听电话耳朵”相符（图4.1 7）。缝合的张力过大是导致此症的根源。在手术时，缝线必须在直视控制下根据要求进行打结，因为软骨管成形过度后矫正相当困难。根据既往的研究结果，对耳轮软骨管成形时最理想的缝合张力是使耳轮与对耳轮调整于同一水平上，或者使对耳轮轻微地高于耳轮平面，这样可确保在去除包扎后获得稳定的手术效果（图4-18）。外耳道狭窄
外耳道狭窄最常发生在手术最后阶段进行耳甲包埋时。在需要做耳甲包埋时务必要防止外耳道狭窄的发生。如果缝合时出现外耳道狭窄，说明耳甲的缝合点过于靠前。此时应毫不犹豫地重新进行缝合，将耳廓后部缝合于更靠后的乳突骨膜。
外耳道的这种后天性狭窄会产生两种功能上的影响：即外耳道皮肤脱屑障碍及高频损失为主的传导性耳聋。治疗则需手术，最常采用的是经软骨外耳道软骨段耳道成形术。

Introduction

耳鼻咽喉科同眼科一样是历史最悠久的独立外科专业之一。从19世纪末起，在学者们的不断努力下，经历了几十年的时间，耳鼻咽喉科终于从普外科中分离出来。1896年，巴黎医院第一代的耳鼻咽喉科医生Marcel Lermoyez在为一部关于耳部、鼻腔、咽部及喉部疾病的治疗系列丛书写序时写道：需要专科治疗的主要原因在于，专科可以通过人体的自然口作为手术径路将损伤减至最低限度，避免大外科引起的较大损伤。”这个前瞻性的想法会在一个世纪以后影响到整个外科的原有格局是普外科医生无法预料的。而在当时，专科治疗与大外科之间的界限仍很模糊。经人类自然口为手术径路的概念使喉科医生敢于在19世纪末最早使用内镜技术，其中最先是食管镜，然后是气管一支气管镜的应用。大约在1930年，首先是瑞士人，之后是法国人Maurice Sourdille开展了耳显微手术，从而使耳鼻咽喉科进入了一个新的时代。
就这样，一个多世纪以来，外科的整体格局已逐渐发生了变化，变得越来越专科化。20世纪，一批耳鼻咽喉科医生由于熟练掌握了术前检查及相关疾病的诊治，敢于将耳鼻咽喉科专业一步步地向普外科领域拓展。而普外科的概念也已被普外科与专科外科相结合的概念所取代。目前微创手术已进人了外科的各个领域。面部、颈部外科已被正式归为耳鼻咽喉科范畴。
同样的，先于其他学科，Marcel Lermoyez的著作中已经包括了内镜及手术显微镜的相关内容。之后，学科与头面及耳神经外科大手术的联系也变得更为紧密。
