Book Abstract

流行病学是一门应用广泛的学科，形成了一套具有严谨逻辑推理性的科学研究方法。流行病学的定义中阐述了流行病学的三阶段研究任务，即揭示疾病与健康状况的流行与分布现象，分析找出流行与分布的规律与原因，在此基础上制定预防控制疾病、促进健康的策略和措施并进行效果评价。不同阶段的研究任务需要运用不同的流行病学方法来实现。
流行病学研究方法有多种分类。按研究性质，可将流行病学研究方法分为观察性研究、实验性研究与理论性研究；按设计类型，可将流行病学研究方法分为描述性研究、分析性研究、实验性研究及理论流行病学研究；按具体的应用目的和研究对象的不同，描述性研究又主要分为现况调查（又称横断面研究）和生态学研究，分析性研究则包括病例对照研究和队列研究，实验性研究则可分为临床试验、现场试验和社区干预试验等。
描述性研究主要是描述疾病或健康状态的分布特征，起到揭示现象、为病因研究提供线索的作用，即产生和提出病因假设；分析性研究主要是起到检验或验证病因假设的作用；实验性研究则用于评价干预措施的效果，证实或确证假设；理论性研究是采用数学模型或计算机模拟，从理论上研究疾病发生、发展与转归的规律。
流行病学往往以观察为基础，首先采用描述性研究，通过对所研究疾病或健康状态及其可能的影响因素在不同时间、空间和人群间分布特征的描述，产生暴露于某因素与疾病或健康状态相关的假设。在获得了研究假设后，流行病学常常运用病例对照研究或队列研究，来观察患有所研究疾病和无该病人群中某些因素的暴露比例，或暴露于某因素与不暴露于某因素的人群中相应疾病的发生率，由此检验暴露因素与疾病发生的关系。在条件允许的情况下，可采用流行病学实验研究对从观察性研究中获得的疾病危险因素实施干预，通过评价干预措施的效果，来验证暴露因素是否是疾病发生的危险因素或病因。流行病学研究的最终目的是提出干预措施以预防疾病的发生，并评价干预措施的效果，效果评价主要采用流行病学实验方法，其中最理想的评价方法是随机对照试验，虽然有时伦理问题不可避免，但其结果最有说服力。
