第一章护理程序
第一节护理程序
护理程序是现代医学模式、护理学发展到一定阶段后，在新的护理理论基础上产生的。新的医学模式要求医疗服务必须由偏重于考虑人的躯体方面的病变转向同时注意患者的心理损伤和反应，由着眼于影响健康的生物因素分析转向重视社会因素的作用。把人看作是个体和心理、个体和社会环境相互联系的一个统一体，用整体的观点指导护理工作。
护理程序一般可分为五个步骤，即评估、诊断、计划、实施和评价。现按五个步骤分述如下：
（一）评估
评估是有计划、有目的、有系统地收集患者资料的过程。根据收集到的资料信息，对护理对象和相关事物做出大概推断，从而为护理活动提供基本依据。评估是整个护理程序的基础。同时也是护理程序中最为关键的步骤。如果估计不正确，将导致护理诊断和计划的错误以及预期目标失败。
1．目的
（1）为分析、判断和正确做出护理诊断或护理问题提供依据。
（2）建立患者健康状况的基本资料。
（3）为护理科研积累资料。
2．内容护士收集资料的内容应该与护理有关，并且尽可能不与其他专业人员重复收集相同的资料。根据人的基本需要层次论的理论观点，评估内容应包括生理的、心理的、社会文化的、发展的及精神的等诸方面的资料，从整体护理观点出发，全面考虑生命过程中这五大方面的资料，从而更好地确认患者的能力及限制，以帮助其达到最佳健康状况。收集资料时一般可从下面14个方面进行：
（1）一般情况：包括患者的年龄、职业、单位、职务、民族、文化程度、宗教信仰、住址、家庭成员、患者在家庭中的地位和作用等。
（2）精神情感状况：患者对疾病和健康的认识，精神及情绪状态，人格类型，感知和辨认能力，患者对压力的反应，对自己目前状况的看法和自我形象概念等。
（3）生殖系统：性功能的状况及有无改变，女患者要询问月经史、分娩史、计划生育情况。
（4）环境状况：患者有无安全感，并根据患者的年龄和精神状况分析是否需要安全保护措施如床栏，是否有交叉感染的环境因素。
