《医古文/全国高等医药院校“十二五”规划教材》分为上编、下编与附编三个部分。
上编为文选。选注医学及与医学相关的古文45篇，包括医家传记、医书序言、医学论文、医案医话、医书凡例、医书提要及医经校记等。以语言规范、词汇丰富、医理清楚为选文原则，并顾及历版教材的传统篇目，以求具有连贯性。按体裁分为四个单元：医家传记（1～10）、医论（11～25）、医著序文（26～36）、杂著（37～45）。同一单元的文章依时代先后排列。正文前都有一个“提示”，扼要说明所选版本、作者生平、著作内容及选文的内容。本教材的“提示”是每一篇、每一章的学习要点，并提供与所属篇章相关的阅读素材，注重检验词语解释、文章理解、翻译、句读及有关基础知识的掌握情况。
下编为基础知识。内容包括汉字、音韵、词汇、语法、训诂、句读、今译、文意理解、古代文化常识等九章。这样安排，是结合高等医药院校本科和医药高职学生的知识结构，通过归纳分析上编阅读文选和中医药古籍的实例，讲清基本概念，阐述基本知识，传授基本技能，提高古医籍水平。所用例句绝大部分出自于教材文选及其他医药相关文章，个别出自中学学过的古文名篇。
附编为《繁简字对照表》与《常用异体字表》。前者按繁体字笔画编排，便于检索，后者列出本教材中的所有异体字以及中医药常用的异体字，便于学习、查阅。

（三）联系医学文史知识而读出内涵底蕴
高本涵曾说：“为了理解汉语文献，必须熟悉汉人的灵魂。”同样，要理解古医籍文句的内涵底蕴，必须联系相关的医药、文史知识，充分认识古人言论的学术、文化渊源和历史背景。
所谓河海一流，泰山一壤，盖亦欲共掖其高深耳。（《类经·序》）
从该则文字可以看出，作者以一流、一土自比，表达了欲为弘扬《内经》理论尽绵薄之力的心愿，这句话的字面义并不难懂。但如果了解“河海一流，泰山一壤”的出处和原意，就会对作者的思想和心态有更深入的理解。“河海一流，泰山一壤”化裁自李斯《谏逐客书》“是以泰山不让土壤，故能成其大；河海不择细流，故能就其深；王者不却众庶，故能明其德。”意思是说一个国家不排斥和挑剔外来人才，才能强盛。可见，张介宾借用这句话是语带双关，除了字面义外，还暗示希望医学界能如河海、泰山般宽容，接受自己的这部《类经》。
若夫《折杨》、《皇苓》，听然而笑，《阳春》、《白雪》，和仅数人，自古如斯。知我罪我，一任当世，岂不善乎？（《温病条辨》）
阅读这段文字，如能了解“知我罪我”的来历，便可更好地领会这句话的深意。《孟子·滕文公下》：“孔子曰：知我者，其惟《春秋》乎！罪我者，其惟《春秋》乎！”汪廷珍以孔子的话劝慰鼓励吴瑭，言下之意说：孔子至圣，撰《春秋》尚有“知我罪我”之叹，你大可不必在意世人的看法。
夫九针者，始于一而终于九，然未得其要道也。（《灵枢·外揣》）
此段文摘“夫九针者，始于一而终于九”的字句，从表面上看似毫无意义的赘言，但实际上大有深意。我国古代文化视数理为一体，数理视理，理寓于数。九针之数亦如此。《灵枢·九针论》日：“一以法天，二以法地，三以法人，四以法时，五以法音，六以法律，七以法星，八以法风，九以法野。”古人以九针配九事，虽是牵强比附，却是借九针之数与自然之数相合的表象，暗寓九针之法与天地阴阳相应之理，这是“始于一而终于九”的第一层意思。在古人观念中，“一”是数之始，用以指称极少极微极精极简的事物；“九”是数之尽，用以指称极多极大极博极繁的事物，九针之法“恍惚无穷，流溢无极”，这是“始于一而终于九”的第二层意思。所以，黄帝实际上是在说：我知道九针之法与天地之理相通，它极精极简而又极博极繁，但是我还没能掌握它的要领。
