张红，天津医科大学眼科中心白内障科主任、教授、主任医师、医学博士、博士生导师。中华医学会眼科学分会委员，中国医师协会眼科医师分会会员，世界白内障医师学会会员。承担多项省部级及国家级课题，发表论著40余篇，主编著作3部，参编著作6部。张红教授是国内最早行白内障超声乳化手术的专家之一，在白内障及人工晶状体方面有很深的造诣，特别是复杂、疑难病例的处理经验丰富，技术精湛。目前已完成白内障超声乳化手术数万例，技术水平居国际先进。曾多次在国内讲学并进行手术表演，受到业内人士的广泛好评。

第二章白内障手术治疗

2.1白内障常用的手术方法有哪几种？

现在白内障囊内摘除术和大切口的白内障囊外摘除术（见图5）已成为历史，白内障超声乳化术（见图6）是目前临床上广泛应用的成熟的手术方式。手术可以通过约2-3毫米左右的切口进行，利用超声乳化仪粉碎吸出混浊的晶状体（见图7），再植入一个有屈光力的人工晶状体（见图8），其显著优点是手术切口小、愈合快、无需缝合，角膜散光小，早期即可获得满意的视力。

2.2白内障手术有年龄限制吗？

随着中国进入老龄化社会和生活水平的提高，越来越多的老年人需要作白内障手术。白内障手术的最大年龄医学上没有统一的限定。有些高龄老人身体素质良好，血压、心脏情况稳定，而且目前白内障手术时间很短，因此多数高龄患者只要全身状况不是很差，都能承受手术。

2.3白内障手术前都要做哪些检查？

为了选择合适的人工晶状体，同时也为了保证手术的安全性及术后效果，白内障手术前需要作充分的检查。包括眼部检查（见图9）和全身的一般检查（见图10）。眼部检查一般包括视力、眼压、角膜曲率、A超和角膜内皮计数，其中角膜曲率和A超检查是为了计算手术中要植入的人工晶体度数。全身检查一般包括血尿常规、肝肾功能、感染疾病筛查、血糖等化验，以及血压、心电图等检查，其目的是要了解病人的全身状况，是否能承受白内障手术。

2.4白内障手术前病人要做什么准备？

白内障手术前准备是非常重要的。除了要配合医生作好一系列眼部和全身检查外，还要作好身心的调整。在术前，病人还要注意休息，调整饮食，戒烟，戒酒，有全身病的患者要在内科医生的指导下，将血压、血糖、心脑血管指标等调整到最佳状况（血压≤140/90mmHg,空腹血糖≤8mmol/l）。术前常规点抗菌素眼药水、冲洗结膜囊。

2.5如何选择人工晶状体？

人工晶状体（见图11）是人工合成材料制成的一种用来替代自身晶状体的特殊透镜。人工晶状体的形状功能类似人眼的晶状体，其重量轻、光学性能好、无抗原性、致炎性、致癌性和生物降解等特性。白内障术后摘除了混浊的晶状体，将人工晶状体植入眼内替代原来的晶状体，使外界物体聚焦成像在视网膜上，也就能看清周围的景物了。
