Author Description

王丽燕，硕士研究生，副主任医师。山西省儿童医院门诊部副主任。1996年7月毕业于山西医科大学儿科系，同年分配至山西省儿童医院工作至今。从事儿内科临床工作近20年。担任山西省医师协会青年医师分会副会长、山西省医师协会儿科医师分会常务委员兼副总干事。专业为小儿呼吸，研究方向为支原体感染、慢性咳嗽。在国家及省级刊物发表论文十余篇。荣获山西省科技厅科技进步三等奖1项。

《小儿支原体感染性疾病》：
第一节肺炎支原体肺炎的临床表现
肺炎支原体感染机体后，临床表现可多种多样。可有从无症状至严重的间质性肺炎的表现。无症状患者比较少见，仅在体检中发现或因其他原因就诊时发现。
一般起病缓慢，病初可有发热、全身不适、流涕、咽痛、头痛、肌痛、食欲减退、乏力等，2～3日后症状逐渐加重，发热可见于80％的患儿，体温波动在37.8～39℃，多在38℃以上，持续时间不等，有的甚至可达2周，可伴有寒战、咽痛，咳嗽多见。咳嗽多呈阵发性剧烈干咳，甚至百日咳样阵发性痉挛性咳嗽，常持续1～3周，偶可延长至数月。咳嗽时多无痰或咳少量黏液痰，个别病例可痰中带血丝或咯血，后期可咳黄痰，部分病例痰多时可出现呕痰现象，呕出大量痰液后咳嗽可暂时缓解。少数患者可伴有胸骨后疼痛、吸气时胸痛。
婴幼儿患者发热多呈中、高热，咳嗽剧烈，常常伴喘促、发绀，多可伴有不同程度的中毒症状，肺部体征重。幼儿可伴有鼻炎、鼻咽炎、声音嘶哑等。年长儿则多为不规则发热、频咳、干咳伴胸痛、咽痛，全身症状明显，而肺部体征不显著，肺外并发症多见。重症患者除高热、头痛、全身酸痛、乏力外，干咳、气促、呼吸困难明显或持续加重，少数可发展为呼吸衰竭，但肺部体征多不明显。
5岁以下感染患儿常有发热、咳嗽，多伴有喘息，病程迁移，胸部影像学表现为支气管肺炎或大叶实变，可出现肺功能下降，易合并其他病原体感染如细菌、病毒等。
肺炎支原体肺炎患儿最常见的症状是发热和咳嗽，有的患儿仅有发热而无咳嗽，有的患儿仅有咳嗽而无发热，但是大部分患儿既有发热也有咳嗽。小婴儿或者有过敏体质的儿童会出现喘息。
