第二节体格检查

体格检查可以发现疾病相关的体征，为疾病的诊断和治疗提供重要依据。性病往往有肛门生殖器部位的症状和体征，但非生殖器部位也可以出现症状和体征。此外，肛门生殖器部位的症状和体征也不全是性病的表现。体检前需向患者说明检查的目的、步骤等，检查需在保护隐私及良好光线下进行，除检查生殖器部位外，也应注意检查全身的皮肤黏膜及系统性体征。男医生对女患者作肛门生殖器相关检查时，必须有女医务人员陪同。

一、男性肛门生殖器检查

1．男性外生殖器检查检查前医生戴一次性无菌手套。让患者取站立或卧姿，暴露外生殖器。

（1）视诊观察：①外生殖器及周围皮肤有无皮疹（红斑、肿胀、丘疹、结节、水疱、糜烂、溃疡、赘生物等）。②阴毛有无阴虱及卵。③有无包茎、包皮过长等。④将包皮翻上，检查龟头至阴茎根部有无皮疹（红斑、肿胀、丘疹、结节、水疱、糜烂、溃疡、赘生物等）。⑤尿道外口有无赘生物、红肿和分泌物，如无明显分泌物，可从阴茎根部向龟头挤压尿道，查看尿道口是否有分泌物溢出。

（2）触诊观察：①阴茎、阴囊有无肿块或触痛，睾丸、附睾、精索有无肿大、结节、触痛。②腹股沟淋巴结有无肿大、触痛。

2．男性肛门及前列腺检查①患者取左侧卧位，将双膝蜷起，用右手朝上拉起右侧臀部，将肛周部位全面暴露。②检查臀部、会阴和肛门周围部位，有无皮疹（红斑、丘疹、结节、水疱、糜烂、溃疡、赘生物等）。③检查肛门有无外痔、皲裂、出血、分泌物、包块、瘘管等。④直肠指诊，检查者戴手套，涂润滑剂，徐徐插入肛门。前列腺位于膀胱下方、耻骨联合后，距离肛门约4cm。正常时大小如粟子，表面光滑、质韧有弹性，中间有浅沟为中央沟。检查前列腺有无增大、结节、压痛等。

3．直肠镜检查①检查前患者应排空大、小便。②患者取左侧卧位。③将润滑剂涂抹到肛门边缘和肛镜的整个长轴上。④将肛镜放在肛门边缘，用头端按摩肛缘，使肛门括约肌松弛，然后轻柔地持续施力沿脐部方向缓缓插入肛门，取出镜芯观察。⑤注意观察直肠黏膜的颜色和质地，有无异常分泌物、溃疡、出血、赘生物及其他损害。⑥检查完毕后，缓慢地拔出肛镜，拔出时检查有无痔疮或其他损害。

二、女性肛门生殖器检查

1．检查前准备检查前嘱患者排空膀胱，而后取仰卧位，将脐部到膝的部位都暴露出来，小腿屈曲，大腿外展。检查者戴消毒手套。

2．检查下腹及外生殖器部位步骤如下：①检查和触诊下腹部，有无压痛、肌紧张、肿块。
