Book Abstract

1968年Berger和Hinglais报道了一组以肾组织系膜区IgA弥漫性沉积为特点、临床表现为血尿和（或）蛋白尿，但不伴有系统性损伤的肾脏疾病，后被称为Bergers病。随后人们发现该病患者肾组织系膜区以IgA沉积为主，常同时伴C3、IgG或IgM‘沉积，其命名也出现多个，如IgA肾炎、IgA-IgG肾病、IgA相关性肾炎和系膜IgA.IgG沉积性肾炎等。如今，经过了近40年的研究，人们对这组疾病有了相当深入的了解，并将其定义为原发性免疫球蛋白A肾病（Primary Immunoglobin A nephropthy，IgAN），简称IgA肾病：一组以肾小球系膜区IgA沉积、同时伴系膜细胞增生和系膜基质扩张为主要病理改变的原发性肾小球肾炎。以往人们曾经以为IgA肾病是一种比较少见的原发性‘肾小球肾炎，但随着对该病认识的深入和肾活检的广泛开展，现在发现该病的发病率并不低，而是全球范围内非常常见的一种原发性肾小球肾炎，在很多地区甚至成为占首位的原发性肾小球疾病，是目前导致终末期肾病（End-StageRenal.Disease，ESRD）的主要病因之一。
我国是IgA肾病的高发地区，如何预防和治疗IgA肾病是肾内科医生不容回避的一个问题，但直到现在，国内对IgA肾病的研究与国际尚有一定差距，对IgA肾病的治疗也没有公认、统一的规范。本书综合近年来国内外有关Iga肾病的临床、实验研究进展及我们的研究和经验，对IgA肾病作一全面介绍，以增进医务工作者对这种疾病的认识，推动我国有关IgA肾病的临床和实验研究进展。

Introduction

IgA肾病是世界范围内最常见的原发性肾小球。肾炎，是导致慢性肾衰竭的重要病因。我国是IgA肾病的高发地区，在原发性肾小球疾病中，IgA肾病占第一位，是导致慢性肾衰竭的首位病因。IgA肾病及其导致的慢性肾衰竭给国民带来了严重的健康问题和沉重的经济负担，如何预防和治疗IgA肾病是肾内科医生面临的重大课题。大多数IgA肾病早期给予恰当的治疗可以改善预后。然而，由于IgA肾病的临床表现及肾脏的病理表现呈多样化，导致不同的医生、不同的医院、不同地区与国家对该病的认知以及治疗方案不尽相同，甚至差别很大，这导致了患者的预后存在很大差别。使临床医生、尤其是广大基层医院的医生产生了某种程度的混乱。在多年的临床工作中，我们深刻感到，临床医生非常需要一本IgA肾病的专著以供他们在工作中参考。我们曾试图找到一本国际公认的指南或IgA肾病的专著，把它推荐给全国广大的肾脏病临床工作者。但迄今为止，对IgA肾病的治疗仍没有国际公认、统一的规范。在许多国家，包括欧洲、美国和我国，尚无IgA肾病的指南，国内也没有关于IgA肾病的专著。为此，我们决定编写一部IgA肾病的专著，并于三年前开始搜集资料、着手编写。
本书综合了近年来国内外有关IgA肾病的临床、实验研究进展及我们的研究和经验，对IgA肾病作了全面介绍。
为使这部：IgA肾病的专著具有全面性、科学性和实用性，具备工具书的作用，我们收集整理了包括1998年以来Pubmed、EMBASE，CCTR和CNKI等有关IgA肾病的基础和临床研究进展的文献报道、Cochrane collaboration登记和发布的RCT研究结果、相关的专业书籍以及业已发布的IgA肾病的治疗指南等。有关IgA肾病的统计数据尽可能采用国际杂志刊登发表的数据（特别是东南亚国家、亚裔人群的资料），其次是国内的数据，最后是作者单位的数据；文献采用直接标注；图片资料除标明出处者外，全部采用单位的图片资料，包括肾脏活组织检查的常规病理切片、免疫荧光以及电镜照片等。
本书主要内容包括IgA。肾病的基础和临床知识及其进展。基础部分讲述IgA肾病的发展史、遗传学、分子生物学、细胞生物学、动物模型、病理生理学及病理学等；临床部分包括IgA肾病的流行病学、症状学、诊断学、治疗学、预后以及儿童、妊娠和肾移植与IgA肾病相关的问题，以及继发性IgA肾病等。另外，作为本书的特色之一，我们根据IgA肾病的主要临床表现及病理改变，将IgA肾病分为八个主要类型，每种类型均有相应的治疗方案，以方便临床医生在临床工作中参考。
