Book Abstract

崩漏是指经血非时暴下不止或淋沥不尽，前者谓之崩中，后者谓之漏下。《景岳全书·妇人规》日：“崩漏不止，经乱之甚者也。”《济生方》日：“崩漏之疾本乎一证，轻者谓下漏下，甚者谓之崩中。”二者虽表现不一，实则病机相同，且常崩漏互见，故总称崩漏。
其主要病机是冲任不固，不能制约经血，使子宫藏泻失常。导致崩漏的常见病因有脾虚、肾虚、血热和血瘀。素体脾虚，或劳倦思虑、饮食不节损伤脾气。脾虚血失统摄，甚则中气下陷，冲任不固，不能制约经血，发为崩漏。如《妇科玉尺》云：“思虑伤脾，不能摄血，致令妄行。”若素体阳虚，命门火衰，或久崩久漏，阴损及阳，阳不摄阴，封藏失职，冲任不固，不能制约经血而成崩漏。或素体肾阴亏虚，或多产房劳耗伤真阴，阴虚失守，虚火动血，迫血妄行，子宫藏泻无度，遂致崩漏，如《素问·阴阳别论》曰：“阴虚阳搏谓之崩。”若素体阳盛血热或阴虚内热，或七情内伤，肝郁化热，或内蕴湿热之邪，热伤冲任，迫血妄行，发为崩漏。若七情内伤，气滞血瘀，或热灼、寒凝、虚滞致瘀，或经期、产后余血未净而合阴阳，内生瘀血，或崩漏日久，离经之血为瘀，瘀阻冲任、子宫，血不归经而妄行，遂成崩漏。崩漏辨证，有虚实之异：虚者多因脾虚，肾虚；实者多因血热、血瘀。由于崩漏的主症是血证，病程日久，反复发作，故临证时首辨出血期还是止血后：一般而言，出血期多见标证或虚实夹杂证，血止后常显本证或虚证。出血期，当根据血证呈现的量、色、质特点辨其证之寒、热、虚、实。
崩漏的治疗，多根据发病的缓急和出血的新久，本着“急则治其标，缓则治其本”的原则，灵活掌握和运用塞流、澄源、复旧的治崩三法。塞流：即止血、暴崩之际，急当塞流止血防脱。澄源：即正本清源，亦是求因治本，是治疗崩漏的重要阶段（一般用于出血缓减后的辨证论治）。复旧：即固本善后，是巩固崩漏治疗的重要阶段（用于止血后恢复健康，调整月经周期）。
