Author Description

郝万山，男，l944年11月出生，毕业于北京中医学院。
现任北京中医药大学教授、主任医师、博士研究生导师。
为北京中医药大学优秀主讲教师、北京市教育创新标兵、国家中医药管理局全国优秀中医临床人才培养专家委员会秘书组成员、教育部课程《伤寒论》创建项目负责人。
在中医药教育岗位上工作近40年，主要讲授《伤寒论》。教学缜密严谨，深入浅出，注重理论结合临床实际，强调讲授中医经典基本知识的同时，更要重视解析其辨证论治的思路和方法。课堂教学生动活泼，条理清楚，注重培养学生思考问题和解决问题的能力。教学评估成绩名列全校前茅。为国家中医药管理局选聘的中医经典著作《伤寒论》示范教学主讲人。
临床重医术，讲医德。既为患者解除病痛，也为课堂教学提供了鲜活的实例，深受学生和患者的拥戴。

Book Abstract

【讲解】
本证成因：第96条和第266条提示太阳病邪传人少阳；第97条提示少阳本经直接受邪，其原因是“血弱气尽，腠理开，邪气因入”，也就是气血虚弱，正气不足，邪气乘虚而入。
主症和病机：往来寒热，休作有时，就是往来寒热，时作时休，这是风寒邪气侵袭少阳经的表现，少阳为小阳，抗邪之力较弱，少阳受邪，正邪分争，互有进退，邪胜则寒，正胜则热，故见往来寒热。胸胁苦满、胁下硬满、胸胁满不去、胁下满等症，皆发生在少阳经脉所过的部位，属邪伤少阳经脉，经气不利的表现。“嘿嘿”是心中不爽快的一种感觉，心中不爽快，自然神情也会抑郁木然，这是由于胆腑气郁，疏泄不利，精神抑郁所致。不欲饮食、不能食，则是由于少阳胆腑气郁，导致脾胃的受纳和运化功能减退所致。心烦是少阳郁火循经上扰心神的表现。喜呕、干呕皆是胃气上逆的表现，少阳病胆火内郁，最容易横逆犯胃，这就是第97条所说的“脏腑相连，其痛必下，邪高痛下”的意思。因为胆热最易犯胃，导致胃气上逆而见呕吐，所以在《伤寒论》中常常以呕吐的存在和不存在，来表示少阳病的存在和不存在。脉沉紧或如第100条所说阴脉弦，紧即是弦，阴脉即是沉取时的脉象，脉沉紧和阴脉弦，皆是脉沉弦的意思。少阳气郁，气血内郁而不能外达，故见脉沉；少阳木气因郁而劲急，致使血脉拘挛，故见脉弦。
或见症和兼证及其病机：邪客少阳，经腑不和，三焦不利，其病变可及于表里内外上下，故少阳病每多或然之证或兼夹之证。胸中烦而不呕，是邪郁胸胁，未犯胃腑；口渴是邪热伤津较重所致；腹中痛为肝胆气郁，横逆犯脾，脾络不和的表现；心下悸、小便不利，属少阳三焦失畅，气化不利，水饮内停，进而水气凌心所致；不渴，身有微热，提示太阳表邪未罢；咳为水饮犯肺，肺气上逆的表现。
以上诸证，其病机总由少阳经腑受邪，枢机不利而致，虽有兼夹之证和或见之证，但在治法上，都应当以和解为主，所谓“和解”，我的理解是“和枢机，解郁结”，或是“和枢机，解郁热”。小柴胡汤是治疗本证的主方。
第97条所说“服柴胡汤已，渴者，属阳明，以法治之”，提示少阳邪气可以传阳明，邪气既然传人阳明，出现了阳明热盛伤津液的口渴，那就应当依照治疗阳明病的方法来治疗了。
【治法】
和解少阳。
【方剂】
小柴胡汤。
【方义】
小柴胡汤共七味药，可以分成三组进行分析。第一组是柴胡配黄芩，柴胡味苦微寒，气质轻清，疏散少阳经中之邪，又有疏理少阳气郁的功效；黄芩苦寒，气味较重，可清少阳胆腑郁火。二药相合，经腑同治，疏清并行，经邪外解，胆热内清，气郁得达，火郁得发，枢机因而条畅通利，针对了少阳病容易经腑同病、容易气郁化火等两大特点，是本方的核心药组。第二组是半夏配生姜，两药皆味辛，以其辛散，可以助柴胡疏通气郁，针对了少阳病容易气郁的特点；又可以和胃降逆止呕，针对了少阳病胆热犯胃，胃气上逆而喜呕、多呕的特点；还可以化痰、消饮、去水，助三焦水道之畅达，针对了少阳病容易生痰、生饮、生水的特点。可见这一组药物的作用，也是很重要的。第三组药是人参、甘草、大枣，这三个药，可以将其看成是半个理中汤，也可以将其看成是半个四君子汤。在治疗发热性疾病的过程中，为什么要用到三个补气的药物？一是因为少阳为小阳，抗邪能力较弱，所以用此三药在柴胡的引领下，可以扶少阳正气以祛邪，有助正祛邪的作用；二是因为少阳之邪容易内传太阴，《难经》和《金匮要略》都有类似的话，“见肝之病，知肝传脾，当先实脾”。这里虽然不是肝病，而是少阳胆病，但肝胆相表里，少阳阳气抗邪不力，邪气也很容易内传太阴，使病证由阳转阴。因此用此三药补太阴脾气，显然有防止少阳之邪内传太阴的作用，可谓治中有防，也算是“治未病”的一种体现吧。
小柴胡汤七味药相辅相成，和枢机，解郁热，达三焦，畅气机，攻补兼施，寒热同调，温而不燥，寒而不凝，而且胆腑清和，则胃能降浊，脾能升清；三焦通达，则水升火降，气通津布，表里之气皆可调和，实是和解之良剂，后世称其为“和剂之祖”。故表里寒热虚实、气血津液阴阳诸病，皆可加减应用。热病用之可解热；郁证用之可解郁；合补药，扶正以祛邪；合血药，行气以活血；合生津药，解热以生津；合利水药，行气以利水；合化痰药，畅气以豁痰；合温阳药，舒郁以通阳；合养阴药，调气以育阴。加减得当，男女老幼，外感内伤，皆可应用，真可谓是左右逢源，被历代医家所珍视。
