早在1918年，日本的高木憲次利用膀胱镜对尸体的膝关节进行观察，从而出现了关节镜。随着众多学者对关节镜的进一步研究，镜视下手术器械及光学仪器的不断产生和高科技手段的不断问世，内镜从而产生。目前内镜在医学界各领域已广泛应用。在手外科，出现了肩关节镜、肘关节镜、腕关节镜、拇指腕掌关节镜、掌指关节镜等关节内的应用，从简单的关节内检查到许多复杂的镜视下手术。但是，内镜在手外科领域关节外应用的历史并不长。1976年，日本的长井仁美分别用空气和生理盐水注入家兔的大腿，进行关节镜在关节外应用的研究，1980年改用Tunnelendoscopy法。直到1986年，奥津一郎发明了固体的透明闭锁性外套管与钩刀组成配套器械，称USE系统，进行了第1例应用于腕管综合征的内镜治疗，从此拉开了内镜在关节外应用的序幕。随后肘管综合征内镜手术、内镜下肌腱移植术、狭窄性腱鞘炎内镜下手术、内生软骨瘤内镜下手术等相继问世。
我院自1995年以来开展了以上手术，并且在顾玉东院士的指导下，通过尸体解剖学的研究，先后开展了胸腔镜下膈神经移植术、健侧颈7移位尺神经内镜下移植术、腓肠神经移植术等。众所周知，微创技术是21世纪外科发展的方向，国内外各种微创治疗正越来越广泛地替代传统的开放性手术治疗。近20年来，内镜在关节外应用的经验已基本成熟，但目前国内尚无一部有关内镜在关节外应用的专著。为此，我们将10余年来积累的一些基础研究与临床应用的经验、体会写成《内镜在手外科的应用》，并特邀腕管综合征内镜手术的创始人奥津一郎先生和日本手外科关节镜手术的专家橋詰博行先生指导和参与《内镜在手外科的应用》的编写工作，并撰写了腕管综合征内镜手术6650例的经验体会、肘关节镜以及透析肩的内镜手术经验。我们愿将《内镜在手外科的应用》与大家共享，以推进内镜在国内的普及和应用。
《内镜在手外科的应用》共分两篇。第一篇介绍了内镜在手外科关节外的应用；第二篇分别介绍了肩、肘、腕关节镜关节内应用的手术入路和基本操作技术；在各章节后附有我院10余年临床与基础研究相关文章。全书从实际应用出发，结合大量的插图和照片，力求基础理论与临床实践相结合，故具有较强的实用性和参考价值。
在编写过程中，我们参考了日本松井宣夫、奥津一郎主编的《整形外科关节镜手册》和兄弟院校的有关材料，在此一并表示感谢。
由于编写时间仓促，水平有限，错误与不足之处，敬请读者与手外科同仁提出宝贵意见。
