三、腰椎间盘突出症
(一)概述
腰椎间盘突出症是在腰椎间盘发生退行性病变的基础上，因负重或脊椎
活动，使椎间盘受到急性或慢性损伤，导致纤维环破裂，髓核突出，刺激和
压迫神经根、血管等周围组织，从而引起以腰腿痛为主要症状的病症。本病
在临床中较为常见，其发病率约为门诊腰腿痛患者的15％，多为20-50岁之
青壮年男性体力劳动者。
腰椎间盘突出症以典型的腰痛及下肢放射性疼痛为特点，属中医学“腰
腿痛”、“痹证”、“腰痛”等范畴。发病部位以腰4、腰5椎之间最多，腰
5、骶工椎之间次之，腰3、腰4椎间少见。腰椎间盘退行性改变与外伤是腰
椎间盘突出症发病的主要原因。中医多从肾虚立论，认为是与肾气不足及腰
膝不坚有关，在肾虚的基础上发病。对本病的治疗，可采用中西药物、针灸
、推拿、牵引等疗法，必要时可进行手术治疗。平时坚持体育锻炼，注意保
护腰部，可预防或减少腰椎间盘突出症的发生。
1．发病原因腰椎间盘是由连接上、下椎面之间的软骨板、中央的胶
质样髓核和围绕周边的纤维环构成的椎体之间的压力缓冲垫，其具有一定的
弹性，可缓冲震荡，并有利于关节的活动。人是直立行走的，腰椎间盘既承
受着人体静态下的压力，又承受着由于生活、劳动时的各种姿势变化所造成
的压力。随着年龄的增长，腰椎经常受到的挤压、扭转等应力的作用和轻微
损伤的积累，腰椎间盘的纤维环和髓核便会逐渐发生退行性改变，其水分减
少、弹性减退、脆性增加，一旦有外伤、劳累、腰部受凉等，便容易造成纤
维环破裂和椎间盘髓核突出。人的年龄在30岁以上，椎间盘即开始发生退行
性变，腰椎间盘突出症的发生也逐渐增多。所以，脊椎的退行性变化是腰椎
间盘突出症发生的内在因素，而外伤、劳累及腰部受凉等为本病发生的外在
原因。
2．中医病机中医认为，腰椎间盘突出症的发生主要与肾虚有关。“
腰为肾之府，肾虚则腰痛”，由于肾精亏损，不能濡养经脉，容易感受风寒
湿邪，致使经脉受阻，气血运行不畅，从而出现腰痛、腰酸等症状。另外，
跌仆闪挫，可致脉络损伤，气滞血瘀，会使络脉阻塞不通，也会发生腰部不
适、疼痛等。肾虚者不但易为风寒湿邪所侵袭，也易出现腰部慢性损伤及闪
挫伤，所以肾虚乃腰椎间盘突出症发生的关键所在。
