第一节病因
一、遗传因素
遗传与哮喘的关系已日益引起医学界重视。早期的研究大多认为哮喘是单基因遗传病，有学者认为是常染色体显性遗传的疾病，也有认为是常染色体隐性遗传的疾病。目前则认为哮喘是一种多基因遗传病，其遗传度约在70％～80％。
多基因遗传病是位于不同染色体上多对致病基因共同作用所致，这些基因之间无明显的显隐性区别，各自对表现型的影响较弱，但有累加效应，发病与否受环境因素的影响较大。所以，若干作用微小但有累积效应的致病基因构成了支气管哮喘的遗传因素，这种由遗传基础决定一个个体患病的风险称为易感性。而由遗传因素和环境因素共同作用并决定一个个体是否易患哮喘的可能性则称为易患性。遗传度的大小可衡量遗传因素在其发病中的作用程度，遗传度越高则表示遗传因素在其发病中的作用越大。许多调查资料表明，哮喘患者亲属患病率高于群体患病率，并且亲缘关系越近，患病率越高。在一个家系中，患病的人数越多，其亲属患病率越高；患者病情越严重，其亲属患病率也越高。汪敏刚等调查了哮喘患儿Ⅰ级及Ⅱ级亲属的哮喘患病率，并与对照组比较，哮喘组中Ⅰ级亲属哮喘患病率为8.2％，Ⅱ级亲属患病率为2.9％，前者的哮喘患病率明显高于后者。对照组的工、Ⅱ级亲属哮喘患病率分别为0.9％和0.4％，其患病率分别低于哮喘组工、Ⅱ级亲属的哮喘患病率。
哮喘的重要特征是气道高反应性。对人和动物的研究表明，一些遗传因子控制着气道对环境刺激的反应。章晓冬等采用组织吸人法测定40例哮喘患儿双亲及34例正常儿童双亲的气道反应性，哮喘患儿双亲大多存在不同程度气道反应性增高，PC20平均为11.6mg／ml，而正常儿童双亲的PC20均大于32mg／ml，说明哮喘患者家属中存在气道高反应性的基础，故气道高反应性的遗传在哮喘的遗传中起着重要的作用。
