Book Abstract

我们通过手术在一只活猪胃部置人一个大的原型装置，包括摄像机、微波接收器、电池光源及一穹窿状铁丝支架以保证组织不遮盖镜头；由于这种内镜的密闭性（真空），有时整个装置可被放于一个透明塑料袋内。手术缝合胃与腹部。设备开始工作后即可见幽门开合。当时这台设备可工作20分钟，每秒获取30帧图像。
尽管现在最便宜的电荷耦合装置（CCD）芯片照相机小到可以吞咽，而且其中一些具有耗能很低的必需元件，但处理器仍然很大——呈扁平形、正方形或矩形——当时我们能买到的最小的处理器为25mm2，其太大，而无法吞咽。我们明白可以将全部元件组装成一个可以吞咽的装置，但必须从医学物理部门的电子处订制。他们同意做，出价7000英镑，这对于当时的我们来说是一大笔钱。

Introduction

近20年来，可曲式视频内镜几乎没什么变化，仅在设计及图像采集上略有进步。但是基本的内镜操作程序相同。可曲式内镜检查并非无创检查，从未横贯组织平面这一点上说，应是微创检查。但因袢的形成而受限，且造成患者不适，大多数患者需要清醒镇静。要做到真正的无创，必须去掉内镜的插入部及导光束等连接内镜与外界的部分。
毫无疑问，胶囊内镜代表了现代内镜操作中唯一最深远的变化，这种独立的可吞咽的图像处理设备在通过胃肠道深部时可无线传输成千上万的高质量图片。吞咽胶囊内镜和吞下一枚较大的维生素胶囊一样没什么不适。
胶囊内镜最初及目前最广泛的适应证是小肠检查，在胶囊内镜出现之前，完整检查这段弯弯曲曲的15～20英尺长的消化道，即使最有耐心、最执著的内镜专家都感到绝望。现在，FDA批准后不到6年的时间，胶囊内镜已经成为我们诊断空肠和回肠疾病的主要工具。
但是胶囊内镜并不局限于小肠，FDA已批准的食管胶囊内镜在发现Barrett食管、食管炎和食管静脉曲张上，可与传统胃镜相媲美。结肠胶囊内镜目前正在临床试验中，可能有益于结肠癌的筛查。可用于胃及十二指肠的胶囊内镜也为期不远了。能取活检或进行镜下治疗的可控制的胶囊内镜目前正在研发之中。
