Book Abstract

如何正确认识颈椎病？
据临床统计，80％以上的颈椎病患者与所从事的职业有关，如电脑操作员、长期处于疲劳状态的驾驶员和文书工作者等，加之坐姿、站姿、卧姿不正常，使颈椎病的发病率逐年上升，且日趋年轻化。而从事司机、IT、电脑设计等职业的人群颈椎发病率更高，达到80％以上。当然，心理因素、医源性损伤、运动等也可导致部分颈椎病。
但是，很多人对自己已患颈椎病麻木不仁。有的是在发病初期症状较轻，没有引起足够的重视，到了病情不断加重，颈部僵硬、疼痛乃至出现上肢或手指麻木时才去医院诊；有的则是因颈部不舒服如酸痛等去按摩，结果越按摩越糟糕，病情愈发严重，才不得已上医院找医生。这除了缺乏基本医疗知识外，还跟患者自己麻痹大意有关。对此，我们对颈椎病一定要有足够的认识，它不但使颈椎及颈部组织本身受害，还影响到心脑血管和中枢神经，造成各类颈源性疾病，可谓牵一发而动全身。
还应值得警惕的是别陷入颈椎病诊疗的误区。一是不要随便请江湖郎中乱按摩颈部。科学的推拿按摩本是治疗颈椎病常用的一种有效的保守治疗方法，对颈型、神经根型和交感型颈椎病有一定的消除无菌性炎症、缓解僵硬和减轻疼痛等作用。但对脊髓型颈椎病，尤其在急性期，就不允许按摩、推拿或牵引，否则会损害脊髓功能，严重者还导致瘫痪。二是病急乱用药。不少患者随意服用止痛药止痛或吃所谓保健品缓解症状，结果收双甚傲，因为药物是靠胃肠吸收，经血液循环才能到达病变部位，而颈椎间盘无血管组织，仅靠软骨板的渗透提供营养，所以服药是“远水解不了近渴”。
另外，应该明白，不少颈椎病之所以难以治愈，除了病情太重、病程太晚外，还因为部分颈椎病确实存在疗效不佳和病情反复的难题。目前治疗该病的三大疗法，手术、微创、保守疗法，每种方法都能治好一部分病例，但哪种方法都不能包治所有患者。而且任何一种疗法对相应的患者都有禁忌证。所以总体治疗原则应当是：能保守者绝不手术；能简单者绝不复杂；能微创者绝不开刀。医生根据病人的病因、病情选择个性化治疗方案，达到效果好、安全性高、损伤小、费用低廉的目的。切忌“千人同法，万人一方”的机械化医疗模式。
当然，应强调，颈椎病若保守治疗半年以上无效，特别是症状严重的神经根型或脊髓型颈椎病，应首选微创疗法，如“射频靶点热凝微创术”，可对病变部位进行温控热凝治疗，使之变性、凝固、收缩，从而解除对神经根的压迫，同时防止病变周围组织与髓核粘连，避免无菌性炎症，并修复破损的纤维环，有效制止复发。该疗法集中了西医学的一些发展成果，体现微创伤、无创口的人性化理念，并结合传统的中医学精髓，实现了开放性手术向闭合性治疗的转变。其优点是：创口小，痛苦少，操作便捷安全有效，并发症少，价格低廉。病情实有必要时，才考虑手术治疗。
