Book Abstract

医生会根据淋巴结是否有肿瘤受累来决定是否行前列腺根治性切除。如果淋巴结中只找到了很少量的肿瘤，有些医生仍会选择行前列腺根治性切除，另一些医生持反对意见。送检的标本将进一步固定处理做成永久的切片并再次进行病理检查。大多数情况下，永久切片和冷冻切片的病理结果是符合的。经会阴前列腺切除术的会阴切口无法切除盆腔的淋巴结，因此还需要做个独立的腹部正中切口或通过腹腔镜的方式切除盆腔淋巴结。对于选择行粒子植入疗法或近距离放疗的患者，需在治疗前取脐下腹正中切口或通过腹腔镜进行盆腔淋巴结切除。
对于高危患者应该切除盆腔淋巴结，因为它直接关系到治疗方式的选择。盆腔淋巴结阳性的比例与前列腺癌分期、PSA水平和Gleason评分密切相关。5%～12%的临床上认为是局限性前列腺癌的患者，术后被证实有盆腔淋巴结转移。因此，在行盆腔淋巴结切除前，应与手术医生讨论如果盆腔淋巴结阳性的话，下一步准备采取什么治疗方案。
盆腔淋巴结切除术的主要风险包括：出血，神经损伤，淋巴囊肿。
（1）出血该术中切除的盆腔淋巴结位于一些较粗大的盆腔血管周围，如髂动静脉周围。损伤了髂血管或者其分支可能造成出血，但极少会出现大量出血甚至需要输血的情况。
（2）神经损伤部分盆腔淋巴结包绕在支配腿部肌肉的闭孔神经周围。如果闭孔神经被切断或损伤但术中能及时发现，只要将其吻合即可，术后对下肢功能影响较小。如果术中未能及时发现，将有可能导致发生损伤的那侧下肢的活动受到一定程度的影响。
（3）淋巴囊肿淋巴囊肿是由于淋巴管损伤造成淋巴液在盆腔积聚而形成。淋巴结被切除时，采用夹闭、结扎、烧灼淋巴管的方法均可减少淋巴漏的发生。淋巴囊肿一般不易察觉，也不会有什么大的危害。但是当体积增到一定程度时，可以压迫周围组织造成腹压升高或腹部疼痛。如果继发感染，可表现为高热、寒战以及腹痛。
