第二节 外科疾病的分类释义
时至今日，外科疾病的分类仍不统一，必须说明的是，疾病的分类不能混同于分科，应以疾病的性质作为标准。《周礼·天官》有“疡医掌肿疡、溃疡、金疡、折疡之祝药剐杀之齐”，既明确了外科当时的范围，也作了最初的疾病分类。《黄帝内经》首先对痈疽作了鉴别，同时也提出许多不同的外科病名。后世医家依自己的见解和经验增人病名，并加以分类，但均不统一。时至明代《外科启玄》又云：“夫疮疡者，乃疮之总名也……疮之一字，所包括者广矣，虽有痈疽、疔疖、瘰疬、疥癣、疳毒、痘疹等分，其名亦指大概而言也。”指出疮疡乃外科一切疾病的总名，其中分类甚多。清《外科心法真验指掌》指出：“疮者，皮外也；疡者，皮内也；痈者，肉之间；疽者，骨之里。”上述可见外科疾病分类之异。正确地进行外科疾病的分类有两个意义，其一有利于探索揭示同类疾病的共同规律，进而提高临床疗效；其二为统一外科病名，奠定基础。每一类疾病，必须阐明其准确的概念，所以释义内容必须掌握。
疡 根据《周礼·天官》的记载，广义的“疡”包括一切外、伤科疾患，如肿疡、溃疡、折疡、金疡。狭义的“疡”则如清《外科心法真验指掌》所指“疡者皮内也”，如痈、疽之类。
疮 疮者，创也。广义的疮，即《外科启玄》云：“夫疮疡者，乃疮之总名也……疮之一字，所包括者广矣，虽有痈疽、疔疮、瘰疬、疥癣、疳毒、痘疹等分，其名亦指大概而言也。”《素问·至真要大论》所说：“诸痛痒疮，皆属于心”之意。狭义的疮，即“疮者皮外也”，凡皮肤外有形可见的各种损害，如起丘疹、疱疹、红斑、皲裂、渗出破溃糜烂等，均可称为疮。丘疹者如粟疮、血风疮；脓疱如黄水疮；红斑如猫眼疮；水疱渗出如湿毒疮；糜烂如水渍疮；皲裂如皲裂疮等。
疮疡 广义指一切外科疾病，如明《外科理例》云：“以其痈疽、疮疡皆见于外，故以外科名之。”狭义是指一切化脓性外科疾病。
肿疡《外科发挥》原注说：“肿疡，谓疮疡未出脓者。”即一切体表外科疾病尚未溃疡的肿块，包括化脓与非化脓性疾患。
溃疡 溃疡一词早见于《周礼·天官》，但无论述。隋《诸病源候论·痈肿久愈汁不绝候》描述道：“脓溃之后，热肿乃散，余寒不尽，肌肉未生，故有恶液澳汁，清而色黄不绝也。”而日《外科发挥》则作了简洁的概述：“溃疡，谓疮疡已出脓者。”即指一切外科疾病溃破的疮面。准确地讲，中医溃疡的概念应该是，凡有脓腐的破溃疮面。
痈 早在《灵枢·痈疽》有云：“营卫稽留于经脉之中，则血泣而不行，不行则卫气从之而不通，壅遏不得行，故热。大热不止，热胜则腐肉，肉腐则为脓。然不能陷，骨髓不为燋枯，五脏不为伤，故命日痈。”指出了痈的意义、特征及发生机制。《外科正宗·痈疽原委论》指出：“故成痈者……其发暴而所患浮浅……故易肿、易脓、易敛，诚为不伤骨易治之症也。”而清《医宗金鉴·外科心法要诀·痈疽总论歌》说：“发于肉脉之间，名痈，属阳。”概括地讲，痈者壅也。气血为邪毒所阻，壅塞不通而发为痈。发于脏腑为“内痈”，生于体表为“外痈”。外痈具有红肿热痛的特征，范围在6～9cm，易肿起、易成脓、易破溃、易收敛，预后良好。
